High Plains Community Health Center is proud to celebrate the grand re-opening of our newly renovated Wiley Clinic! 🎉

We are excited to continue serving the Wiley community with improved facilities and expanded access to care, including Primary Care, Behavioral Health, and Dental Hygiene services, bringing more healthcare resources closer to home.

We are incredibly grateful to everyone who joined us in celebrating this milestone and to Jake Specht and Bison Enterprises, LLC, for their hard work and partnership in bringing these improvements to life. Thank you to our community leaders, partners, and Wiley residents for your continued support!

Monday: 8:00 a.m.–12:30 p.m. and 1:30–8:00 p.m.

🎉 Exciting News for High Plains Community Health Center and Southeast Colorado!
High Plains Community Health Center has been awarded $2,114,713 through Colorado’s Rural Health Transformation Program!

This investment will help us expand access to healthcare, strengthen our workforce, and continue bringing high-quality, integrated care closer to home for the communities we serve.

Through this funding, High Plains will be able to:
• Expand access by recruiting additional medical and behavioral health providers
• Increase behavioral health and substance use services
• Launch pediatric dental services, including three new pediatric dental operatories
• Expand chronic care management and remote patient monitoring for patients managing conditions such as diabetes and hypertension

This funding will support our continued efforts to improve appointment availability, reduce wait times, increase patient capacity, and make sure residents throughout Southeast Colorado can receive the care they need right here in our communities.

We are incredibly excited about what this means for the future of healthcare in Prowers, Bent, Baca, Kiowa, and surrounding communities. ❤️

✨ Provider Spotlight: Liesl Wyka, NP, CMO ✨

We are proud to recognize Liesl Wyka, NP, CMO, a dedicated provider and clinical leader at High Plains Community Health Center!

Liesl graduated with her Master of Science in Nursing from the University of Colorado Colorado Springs (UCCS) in 2013 and is Board Certified in Family Medicine. She also holds a Bachelor of Science in Education from The Ohio State University and is a Certified Autism Specialist.

As both a provider and clinical leader, Liesl believes in partnering with patients so they can feel as well as they can, physically, mentally, and emotionally. Her approach goes beyond simply treating illness. She is passionate about supporting long-term wellness by understanding what matters most to each individual.

That means listening, setting shared goals, and connecting patients with the right care, knowledge, and resources to help them stay healthy and well.

🌟 Provider Spotlight: Dr. Sonia Grewal, DDS 🌟

This week, we're excited to shine the spotlight on Dr. Sonia Grewal, DDS, a valued member of the High Plains Community Health Center dental team for the past 17 years! 🦷💙

Dr. Grewal earned her Doctor of Dental Surgery degree from New York University College of Dentistry in 2008 and joined High Plains Community Health Center shortly after completing her degree. Since then, she has dedicated her career to caring for individuals and families throughout our community.

Dr. Grewal is passionate about helping patients achieve healthy, confident smiles through personalized, compassionate dental care. She believes in getting to know her patients and providing care tailored to their individual needs and goals.

After 17 years at High Plains, Dr. Grewal has become an important part of the team and our community. We are grateful for the years of dedication and exceptional care she has provided to the families we serve!❤️

This week, we're excited to introduce Dr. Helen Iams, MD, the newest addition to the High Plains Community Health Center Main Clinic team!

With more than 20 years of experience in medicine, Dr. Iams is passionate about partnering with her patients and empowering them to become active participants in their own healthcare. She believes that great healthcare is a partnership—one where patients feel heard, informed, and involved in decisions about their health.

🏃‍♀️ Dr. Iams also brings extensive training in sports medicine and enjoys working with athletes of all levels—from experienced competitors to those who are simply getting started. Whether you're pursuing a personal fitness goal, managing a sports-related concern, or looking to improve your overall health, she is passionate about helping patients stay active and healthy.

We are thrilled to welcome Dr. Iams to High Plains and to have her experience, knowledge, and patient-centered approach available to the families of Southeast Colorado.
