Mild cases of positional often improve with consistent repositioning, supervised , and physical therapy, especially when these strategies begin before 4–5 months of age. As babies gain head control and spend less time lying in one position, some natural remodeling can occur.
However, research shows that moderate to severe plagiocephaly frequently does not fully correct on its own. In one study of infants with severe flattening, 66% showed little to no improvement after two months of natural observation. The window of rapid skull growth slows after 6 months, so the opportunity for significant spontaneous correction becomes more limited with each passing week.
