This is an interesting case involving extraction and re-implantation of a failing Maxillary First Molar Tooth #3. A patient presenting with failed root canal therapy causing pain and swelling and resulting in an inability to function. We extracted the tooth and identified the affected root structures causing the infection. The infected elements were removed under magnification and repaired chairside. After which, the infected bone socket was debrided using a clawed surgical spoon and sterile saline and the tooth was re-implanted into the socket. Bone graft particulate was then packed in and around the bony defect and the patient was released for 2 week follow-up. After a month of healing and confirming a resolution of symptoms and infection, a new zirconia crown was fabricated using our state of the art CEREC 3-D Prime Scan and Prime Mill intraoral system. This tooth was given new life. This level of patient care is rewarding and quite satisfying. Although there are many instances where teeth can not be rehabilitated, it is my professional belief and commitment that our natural teeth should be repaired and retained when ever possible. We had the pleasure of placing a dental implant for a missing mandibular right first molar #30. We utilized a Nobel Biocare Active Dental Implant and completed the surgery with a healing abutment. Here, I surgically augmented the gingival tissue scallop to more intimately fit around the abutment to allow for accelerated and predictable healing. We will see this patient back in 2 weeks to remove sutures and evaluate healing and then again in about 12 weeks to fabricate the crown restoration using our CEREC 3D scanning and milling for a same day crown restoration. It's always exciting and rewarding having these opportunities to provide this level of care which helps our patients keep smiling. Smiles Ahead!

Sometimes a diseased tooth can be removed, repaired and re-implanted. This morning our patient presented with a buccal swelling and infection associated with a lower molar tooth 18. It was identified as prior failed root canal therapy. Our objective was to remove the tooth and either attempt to repair the tooth or bone graft remaining socket for an eventual dental implant We removed the diseased tooth and a large cyst/infection came out with the roots and the socket was carefully debrided and the cyst and infection was removed from the roots. The tooth was inspected and the problem was identified at the root tips. These areas were cleaned and treated chairside. A retro-fill product was placed into the defective root tips and the tooth was re-implanted into the socket. After which, I placed a bone graft particulate around the roots where the the large bony defect which the cyst had created. We will follow up with this patient in a few weeks. I have had great success providing this tooth saving tx over the years. Sometimes it's better to save the teeth we were given! Smiles Ahead!

We had the privilege of treating a gentleman with a missing left maxillary first molar who presented with inadequate bone for a conventional dental implant. This case is an example of a maxillary sinus lift in conjunction with implant placement. We used a Bicon 6x6 Short implant after raising the floor of the sinus and placing bone grafting. Another exciting case moving towards completion. This patient presented with an ectopic maxillary right cuspid in the central hard palate. We surgically uncovered the tooth and placed a button and chain appliance and began the long and arduous process of erupting and repositioning this important tooth into it's proper location. Initially, it required recapturing the collapsed lost space for the cuspid via expansion and coil spring mechanics. Once this was completed I fabricated a specialize palatal wire appliance with various shapes (photo) to tie elastic thread for optimal leverage as the tooth began emerging. Today, we removed the palatal wire appliance and placed a bracket on the cuspid and have tied that utilizing power chain to begin the rotation and displacement into the arch. This treatment has only been possible because of the amazing patient and his incredible and tolerant disposition. What a privilege for our team and I to be able to treat this patient and provide this multi-modality orthodontic and surgical solution. Smiles ahead!
