3 things that make splint delivery smoother:

1. Specify the objective.
Stabilization, deprogramming, repositioning, ramp guidance, parafunctional control โ€” these are different clinical goals. True Splints support a range of designs. The design should follow the objective, not the other way around.

2. Send complete records.
A clean digital scan is the start. For splint cases, also include bite registration, photos when useful, starting jaw position, vertical notes, material preferences, and any delivery or follow-up instructions.

3. Plan the follow-up.
The splint case doesn't end at delivery. Know what to check at seating, when to re-evaluate, and what to document over time.

When the prescription is specific, the lab team can build for it.

TrueDorsal is not "just another oral sleep device."

The TrueDorsal is a dorsal-fin-style sleep device that uses upper and lower trays with fins to support advancement while allowing opening, closing, and freedom of movement. That makes it useful to consider when tolerance and comfort are major parts of the case plan.

Where it sits in the lineup:
TrueDorsal is comfort and movement-oriented. Helpful when a patient may not tolerate feeling locked in.
TrueHerbst is more retention and durability-oriented. Helpful when staying engaged overnight is the priority.

The point is not that one device is always better. The point is that each device solves a different problem.

Most TMD device adjustments at delivery trace back to something upstream, usually to an unclear diagnosis or a bite registration taken before the diagnosis was made.

When the history, exam, classification, and registration technique are dialed in for that patient, the rest of the therapy becomes much more predictable, and the chair time on delivery day becomes much shorter.

That's exactly what Dr. Mayoor Patel and Dr. Terry R. Bennett teach in their 2-day Concord Seminars course at the Craniofacial Pain Center of Georgia in Atlanta. June 12 and 13.

Sometimes the device isn't the real problem.

Sometimes the nose is left out of the treatment plan.

In oral sleep device therapy, nasal breathing can make or break tolerance and outcomes. If the patient can't breathe well through their nose, the oral device may be doing its job, but the airway pathway is still fighting against resistance.

That is why case planning should look beyond the device itself.

Three takeaways from this webinar clip:

1) Ask about nasal breathing early.
Patients may not volunteer it unless you ask directly. โ€œCan you breathe through your nose at night?โ€ is a simple question that can change the direction of the case.

2) Look for signs that support the conversation.
Dry mouth, mouth breathing, a scalloped tongue, a history of congestion, allergies, deviated septum, and difficulty keeping the lips sealed can all be worth documenting.

3) Build a collaborative pathway.
Oral sleep device therapy may still be appropriate, but some patients also need support from ENT, allergy, myofunctional therapy, medical management, positional therapy, or CPAP coordination.

Device choice still matters, too. But no device should be chosen in isolation from the patientโ€™s breathing reality.

What to do this week: add one nasal breathing question to your intake and one nasal observation line to your case notes.

We make a lot of TMD devices.

What we've learned, watching cases come through the lab for 27 years: the device that's true to fit and function on the first delivery is almost always the device that started with a clear diagnosis and a careful bite registration.

That's why we're behind this one.

Usually, it's fabrication; it's workflow.

In many cases, the friction starts before the device ever reaches delivery day.

Incomplete records can delay the case.
