Technically it’s an overlay but we will call it an onlay for ease of discussion.

Cemented with heated APX composite. A bit more technique sensitive bonding with composite as opposed to resin cement.
The filler amount is increased so the strength, fracture resistant, and bonding is better, among many other things like film thickness and voids. 🫡

Slide 2: Not a terrible starting point looking from the outside. However her front crowns had 1mm gaps on the lingual and the posterior implant crowns are not seated on the abutments correctly. In addition, her posterior crowns are missing porcelain on the chewing surface with metal showing. What’s this mean? She is a bruxzer and her bite has compensated for the wear- the front teeth have positioned themselves in a flared position.

Ideal treatment would be to do all her top teeth and open the bite. This would allow us room in the anterior to tilt the teeth back into proper inclination and rotate the posteriors inward.

Finances dictated the amount of work we could dive into.
We opted to do #6-11 with crown lengthening and the lower canines.

#6 was an implant with a white abutment, her other stump shades were A3. Getting a harmonious color blend provides a challenge when we are using natural porcelain restorations.

Fortunately our office has multiple try-in paste colors and cements - we utilized A2 shade for the implant and a translucent shade for #7-11.

When implants aren’t feasible, we can do bridges.
Tissue molding was performed after extractions to create papilla.

Patient requested the “whitest shade possible”.
Facial texture was added to create visual depth, translucency was blended to break up the white color. Making the difficult look easy is what we excel at.

This case demonstrates that subtle tweaks can yield great results. Not every case requires a change of color- sometimes it requires a gum lift, black triangle closure and proper crown/tooth orientation.

We opted to keep the same shade and material choice, but reinvented the shapes while also cleaning up cavities.
She looks like a rejuvenated version of herself.
