This is the kind of case that shows why dentists trust us (Dr. Nima at Evergreen Endodontics) with their most challenging root canals.

This upper second molar was basically the final boss of root canals.

First, it sits all the way in the back of the mouth, where visibility and access are limited. Its roots were also about 25 mm long, nearly a full inch, which puts them among the longest we commonly encounter.

Then came the real challenge. One canal had a double S curve. Instead of following a relatively straight path, it curved in one direction and then sharply curved back again. Guiding tiny instruments through that anatomy without straightening the canal, creating a ledge, separating an instrument, or damaging the root takes patience, precision, and a very light touch.

We completed the treatment through the patient’s existing crown, preserved the crown, and carefully followed the original canal anatomy all the way to the ends of these unusually long roots.

Imagine being in so much pain that you can barely think straight. 🤯

That was the situation for this patient, who was referred to me by his dentist with excruciating pain from a lower left molar. His dentist told us he was in absolute agony, so we made sure to see him that same day.

Once treatment began, we discovered an especially challenging root canal system. The roots were extremely long, measuring about 25 mm, and the canals were very narrow, delicate, and curved, particularly in the mesial root.

Cases like this require patience, precision, advanced magnification, and the ability to carefully navigate areas that are difficult to see and reach.

Despite the complexity, we were able to treat the tooth and address the source of his pain.

For patients, severe pain does not always mean a tooth has to be removed. In many cases, timely treatment can relieve the pain and help save the natural tooth.

When a patient is in serious tooth pain, the goal is simple: get them comfortable and help save the tooth when possible.

This patient was referred to emergency dental endodontic specialist Dr. Nima at Evergreen Endodontics in Issaquah for an emergency root canal because the case was more challenging than usual. The tooth already had a crown, the roots were extremely long, about 26 mm, a little over one inch, and the canals were very thin and slender. One of the roots also had a curve near the end.

That may sound like a small detail, but it matters. Long, narrow, curved roots can make root canal treatment much more delicate. In cases like this, careful planning, magnification, and experience help us clean and seal the tooth as safely and predictably as possible.

Most patients never see these details on an X ray, but dentists do. That is often why a general dentist may refer a patient to an endodontist, especially when a tooth is painful, previously crowned, unusually long, curved, narrow, or difficult to access.

Some teeth are straightforward. Some are built like a maze and a roller coaster.

This patient came in to “Evergreen Endodontics” for an emergency visit with real tooth pain, and it wasn’t random. Two teeth had a history of deep decay that had been treated with fillings, but the original cavities were close to the nerve, and over time both nerves became inflamed. His dentist saw long, sharply curved roots on the X ray and referred him to Dr. Nima and our endodontic team here in Issaquah.

Before we started the root canal, we used 3D imaging (CBCT) to map the roots. It revealed an extra hidden canal that can be missed on a standard X ray, and that can lead to lingering symptoms later.

Check out this challenging case! This patient came in with a crown on tooth number 30, and one of the canals (the Mesio Buccal) was calcified due to the canal being too close to the crown margin. A 3D CBCT scan and a microscope were essential to locate it. The tooth was also exceptionally long... over an inch (25 & 26 mm roots) from crown to root tip, placing it among the longest we’ve seen for a molar. We successfully completed the root canal, relieved the patient’s pain, and wanted to show you the kinds of complex cases we handle. This patient felt no pain. She wasn’t sure why her dentist sent her to us.

A 3D scan showed the truth: internal root resorption. The tooth’s canals were eating away at the inside, leaving the walls paper-thin. Think of a pipe corroding from the inside out, except it was her tooth.

We don’t know how long it had been happening or how quickly it was progressing. Left unchecked, the tooth would eventually fracture or need removal. Choices were:
1. Monitor and accept eventual removal
2. Try root canal therapy now to stop the inflamed tissue causing the damage

She chose treatment. The canals were distorted and fragile, but we carefully cleaned, sealed, and rebuilt the tooth with a core that gave it more strength.

For now, her tooth is saved.

TOOTH SAVING SUCCESS STORY!

As an endodontist, I've seen my fair share of teeth in trouble. But recently, I was thrilled to witness the transformation of a tooth that had been struggling for years!

Tooth #31, which previously underwent a root canal, many moons ago in another town, had developed an abscess. The left side picture shows you what it looked like PRE-operatively - unfortunately, there was significant bone loss around the area.

BUT, thanks to our non-surgical endodontic retreatment, we were able to fix the problem and give the patient a second chance at saving their tooth!
