Buccal access. Necrotic Pulp. Symptomatic Apical Periodontitis. Abfraction lesion resulting in caries. RPD abutment. Asymptomatic at the second visit. The amalgam was removed just to make sure nothing was going on underneath (there wasn’t, and no dreaded cracks on the enamel). All other mandibular premolars also had abfraction lesions but are still vital; recommended immediate restoration of those teeth.

Dual access and Radix. Symptomatic Irreversible Pulpitis. Mesials were surprisingly calcified in the apical third. Tempted to open up the radix but stopped short with the 17.04. In retrospect the 17.04 would have went to length without issue and then the postop rad wouldn’t be so bothersome. 😂

Conservative Endodontics.

SIP

ER visit: severe hyperemia. Patient was asymptomatic immediately after this first visit.

Second visit: still asymptomatic…until the canals were re-instrumented. Two middle mesials were hiding pulp tissue remnants in the apical few millimeters.

Also there was a radix entomolaris.

Patient went straight to the referring dentist for crown placement.

Sinus tract on the buccal.

Hairline crack on the mesial marginal ridge. 

Gold overlay with an underlying hairline crack on the distal. 

Mild flareup after the first visit and the sinus tract resolved by the time of the second visit.

Gentlewave at the second visit.

Radix entomolaris.

Full coverage gold crown to be made.

Cracked tooth.

Symptomatic Irreversible Pulpitis due to a debris-housing crack.

Immediate dentin seal at the first visit. ✔️

Tooth was asymptomatic at the second visit. ✔️

The agony of the postop radiographs. 😭

Not yet done…there was gutta percha squirting out into the sulcus so we removed the distobuccal composite, excess gutta percha and recurrent caries.

Now we’re done. 😅

Distalized and Opportunistic Access

Partially Vital/Partially Necrotic Pulp. Placed a permanent pre-endo buildup with the matrix.

The patient was asymptomatic at the second visit. At this time, we removed the recurrent mesial caries on Tooth #31(47); would’ve been wiser/easier to perform this before placing the permanent pre-endo buildup on Tooth #30(46). 😭

Next step is a crown on Tooth #30(46) and completion of the caries removal and restoration of Tooth #31(47).
