When you give the papilla room to breathe.
A challenging case of failing #8 and #9 due to failed RCTs and compromised sockets—managed .us with two immediate Neodent implants and immediate load.

By controlling the contact point and the emergence profile (Zone E, Meda et al. 2021), we achieved complete papilla fill and a favorable apical repositioning of the gingival margin.

This week we hosted our third study club event here in Houston—bringing together some of the city’s top dentists and specialists. It’s incredible to see such a talented group of clinicians gather under one roof, leave egos at the door, and openly share challenges while embracing a multidisciplinary approach to elevate patient care.

This patient had a pterygoid implant placed after a wisdom tooth extraction as part of an AOX case. Implant was rock solid — but the soft tissue told a different story. 😤

About 3 months post-op, a sinus infection flared up. An earlier attempt to close the sinus was only partially successful — patient could still feel water entering the sinus when using a Waterpik. 😬

Now, inspired by the legend himself .g.dmd.omfs (Juan and Done), we went back in with a staged approach:

Switched patient to provisionals
Trimmed pterygoid extension and relieved intaglio
Removed MUA on the pterygoid and placed a cover screw for submerged healing and gain of keratinized tissue
Partial dissection to preserve as much native tissue as possible
Passive flap closure with buccal fat pad support for a solid double-layer seal

Let the healing begin. 👊

A huge thank you to the incredible students of the Australasian College of Dental Practitioners for their dedication and commitment to this two-year implant diploma. Many of you have made significant life adjustments—leaving families, taking time off work, and investing in your education—to advance your skills, and it’s been an honor to be part of your journey.

Amazing world class faculty .prosthodontist .davidattia .varun_prosthodontist , whose expertise and passion continue to elevate this program. And a big thank you to the organizers Brooke, Magda, and Brett for making everything run seamlessly.

Over six intense days of lectures and hands-on training, we covered the foundations of implantology, diving deep into treatment planning, bone augmentation, soft tissue management, and sinus lifts—key pillars of successful implant dentistry.

Pterygoid revision

Had started this case of 3 months ago PATZI protocol was done to overcome an atrophic maxilla. 2 Pterygoids 1 zygoma 1 trans sinus and 2 anterior nasal crest implants.

At the time of second stage the left pterygoid implant was mobile but not painful. After CT scan evaluation it looked like the implant was integrated by a 2mm only.

Today we did a revision on that implant via flapless pterygoid taught to me by .g.dmd.omfs . pterygoid implant placed with above 60Ncm torque.
