To our Chace Lake Family
I would like to remind you that your health and safety are our top priority. We have always worked to create a sterile environment for every appointment. While considering the COVID-19 virus, I want to assure you that we are taking additional measures in maintaining a safer atmosphere. We are committed to continue treating your oral health while making sure you feel safe while doing so. The Chace Lake Family Dentistry team is very appreciative of your cooperation and understanding during this time.
With your help, the following suggestions will help us maintain a safer environment:
-If for any reason you feel uncomfortable about your upcoming appointment, please call our office to notify us. We understand this time of uncertainty and only ask you let us know rather than not showing up for your appointment.
-If you are recently been sick or experiencing any fever, coughing, shortness of breath, sneezing, sore throat, sinus or respiratory symptoms, please call our office to discuss your appointment.
-If you or anyone with you have been on a cruise, traveled outside the United States within the last 21days, we ask that you reschedule your appointment until a later date. We truly appreciate your dedication to us, but please consider the heath of others if you fall under any of these circumstances.
-To follow the guidelines of social distancing, we have closed our waiting room and we ask that only the patient come inside to the appointment. We would greatly appreciate it if all other parties remain outside or in your vehicle. Exceptions being a parent accompanying a minor or a caretaker of any patient with a medical need. We do ask that you limit this to one parent or caretaker.
If you have an upcoming confirmed appointment -
Our waiting area and front desk are closed to public. Please call our front desk 205-989-0090 upon arrival and we will direct you from there and let you know when your room is ready for your appointment. We will our best to keep your exact appointment time but please understand that we are taking additional measure to maintain the safety of us all.
If there are any changes in our schedule and/or procedures we will update you.
Thank you,
Your Chace Lake Family Dentistry Team
