We spend a lot of time talking about “natural esthetics” with our patients…because patients are people, not just a set of teeth attached to a skull. We use all kinds of brilliantly white, diamond-sparkly materials to repair and replace tooth structure, but the reality is most folks just want to look like themselves again, not something entirely different.

This patient wanted to restore her damaged lateral incisor, which had a huge variety of textural and optical variations throughout its natural structure (as do her other natural teeth). The most “natural” choice in this case was to recreate these details prosthetically rather than erasing them and creating a visual contrast. Using technology, Ivoclar ceramic, and a little artistry, we fabricated a crown for this tooth in a SINGLE VISIT that disappears against the backdrop of her neighboring teeth. Truly natural.

Ah, the trusty ol’ PFM crown margin - both reliably sturdy and predictably ugly! Porcelain-fused-to-metal crowns (PFMs) have been around since the 1950s and were an incredible advancement for dental esthetics back in the day when “tooth colored” materials were always extremely fragile. Their main esthetic drawback has always been the high-contrast “dark line” margin where their metal edges meet the natural tooth structure supporting them. Despite being a mainstay of general dentistry for decades, they have been completely upstaged by major advances in durable dental ceramic materials in recent years. Nowadays, a ceramic crown can be fabricated for you in a SINGLE VISIT that is (1) more fracture resistant than PFM, (2) more naturally esthetic than PFM, and (3) more affordable than a PFM’s precious metal alloys.

This patient had a very conspicuous PFM margin on her upper left canine tooth that made her smile look unbalanced and disproportionately short on the affected side. We used technology to fabricate an Ivoclar replacement crown in a single appointment and drastically improved the symmetry and proportions of her smile. The dark line is no more!

What better way to conclude your work week than appreciating a smile overhaul?

Typically, the feature point of a smile is a patient’s maxillary central incisors (aka upper front teeth). Ideal central incisors need to be (1) anatomically congruent with the adjacent lateral incisors, (2) similar to or slightly brighter than adjacent teeth with respect to color and translucency, (3) proportionately wider than the lateral incisors, and (4) proportionately longer than the lateral incisors by 0.5 to 1mm. Variations abound, but these general esthetic principles tend to yield pleasing visual results when it comes to yer pearly whites!

This patient’s central incisors had a less-than-ideal grade in many respects (symmetry, anatomy, color, proportion), BUT we overhauled them both in a single visit with crowns, and now his smile is indeed a feature, not a bug!

Does one of your front teeth look…different?

Maybe not even that bad, but just different?

Asymmetrical contrast is a visual magnet when it comes to facial esthetics. Not everybody can tell you why something looks less-than-pleasing, but eyes are always drawn to contrasts like these. This patient wanted to improve the color and contour of her “different” central incisor, so we bonded an Ivoclar composite resin veneer into place and watched the contrast literally disappear. No anesthetic, less than an hour, contrast gone.
