Legacy Endodontics (Wade R. Hirschman DDS MS) Privately owned Endodontist (Root Canal Specialist) Dr. Hirschman is dedicated to the field of Endodontics and quality patient care.

Dr. Hirschman started work as an Endodontic dental assistant at the age of fourteen for his father, Dr. Louis Hirschman. Dr. Wade Hirschman is now a third generation Endodontist in his family along with his older brother, Dr. Bradley Hirschman, who currently practices in Novi, Michigan. The Hirschman family endodontic lineage started with Dr. Harold Maxmen, one of the original pioneers of Endodontics in the 1930's.

A tooth that has “died” too early doesn’t always mean it’s lost forever.

This is a great example of how we can sometimes help a young tooth continue developing even after it has become infected.

A 9-year-old patient came to us with an infection in a lower tooth that had not yet finished forming its root. Because the root was still developing, our goal was not just to clean the infection—but to encourage the body to continue natural healing and development.

We carefully disinfected the inside of the tooth and placed a medication to allow the area to calm and heal over the next couple of weeks. At the next visit, the medication was removed and a gentle bleeding response was created inside the tooth to form a natural scaffold for healing cells.

We then sealed the tooth with a strong bioceramic material and placed a permanent restoration.

At the 1-year follow-up, we were happy to see continued root development and healing exactly as expected.

Last week, Dr. Hirschman attended the American Association of Endodontists annual meeting in Salt Lake City, where leading specialists from across the country gathered to explore the latest advancements in endodontic care, technology, and clinical innovation.

Patient Case Monday! 🦷

When infection affects multiple teeth, the right diagnosis and treatment plan can make all the difference.

In this case, a patient had an infection involving several upper teeth—some of which had previous root canal treatment that hadn’t fully resolved the issue. Using advanced 3D imaging and a comprehensive evaluation, we were able to identify the source of infection and create a targeted approach to save the natural teeth whenever possible.

Treatment included:
- Root canal therapy to address complex anatomy
- Microsurgical treatment (apicoectomy) to remove infection at the root level
- Careful management of prior treatment challenges

The result?
At 6 months: clear signs of healing and bone regeneration
At 1 year: continued bone recovery and successful outcomes

Keeping Smiles Healthy, Even in Complex Cases! 😁

This patient was missing her lateral incisors ( #7 and #10) since childhood. Years ago, her dentist transplanted tooth #12 (a premolar) into the area of the maxillary canine ( #11) to help maintain proper function and aesthetics. The transplant healed successfully, but over time the pulp canal system of #12 became traumatized, developing calcification and leading to infection.

Because the canal was so calcified, a nonsurgical root canal wasn’t possible. With a poor crown-to-root ratio (more tooth structure above bone than root within), our team performed a conservative apicoectomy, sealing the root tips to eliminate leakage and infection.

Outcome: Infection resolved, tooth preserved, and her bridge remained intact!
