If I could change one thing about how parents handle their kids' dental care, it would be this: stop letting four-year-olds brush unsupervised.

I know β€” independence is good. You want them to learn. And your child probably loves brushing and does it with real enthusiasm. But enthusiasm doesn't equal effectiveness, especially when it comes to the back molars.

A four-year-old does not have the dexterity to get a toothbrush all the way to the back of their mouth and clean those surfaces properly. They're doing a great job on the front teeth β€” the ones they can see and easily reach β€” and missing everything behind them.

You tell me your child brushes independently and loves doing it. And then I look in their mouth and find plaque on every single back molar. Your child isn't doing anything wrong β€” their hands just aren't physically capable of that range of motion yet.

Let them hold the brush and let them try. Then you go back in and finish the job on those back teeth. You're not taking away their independence β€” you're covering the spots their little hands can't reach.

I'm a pediatric dentist in Glendora, and I'd rather your kid eat ice cream than goldfish crackers.

I know that sounds ridiculous, but stay with me. The biggest factor in cavities isn't how much sugar your child eats β€” it's how long that sugar stays on their teeth. Skittles, gummy bears, fruit snacks, cake pops β€” they all stick. They wedge themselves into the grooves of your child's molars and sit there feeding bacteria for hours.

Ice cream? They eat it, drink some water, and it's gone. It doesn't stick around. It doesn't camp out in the grooves waiting to cause problems.

Same thing with crackers versus crunchy vegetables. That goldfish box has a cute fish on it and says "organic," but those carbs break down into sugars that cling to teeth exactly like candy does. A carrot or a cucumber actually helps clean the teeth while your child chews.

I'm not saying throw out all the snacks β€” I'm saying be mindful of the sticky ones, because those are the real troublemakers.

It's ten o'clock at night and your child just fell off the bed and knocked a tooth loose. Your brain is racing β€” is this an emergency? Should you go to the ER? Should you even bother calling the dentist at this hour?

Call us. We have an after-hours emergency line for exactly this reason.

Maybe it's urgent and we need to see your child first thing in the morning. Maybe it's something that can wait until the next available appointment. Maybe all you need is someone to tell you to give them children's ibuprofen and keep an eye on it overnight. Either way, I'd rather you call and find out than sit there googling "loose tooth emergency" at midnight and spiraling.

We screen every after-hours call and walk you through what to do. If it's truly urgent, the person on the line will reach me directly and I'll call you back with a plan.

You're not bothering us and you're not overreacting. There are no stupid questions β€” if it feels like an emergency to you, that's enough reason to pick up the phone.
