L-PRF is a biological scaffold composed of a dense high cross-linked fibrin mesh that entraps platelets and leukocytes. This bio-membranes can release growth factors, adhesion molecules, and pro- and anti- inflammatory cytokines.

L-PRF stands for Leukocyte-Platelet Rich Fibrin. This bioactive “band-aid” is created from patient’s own blood and then placed in the surgery site to promote healing.

The blood specimen is collected just like in routine blood test. The specimen then spun in dedicated centrifuge system to separate and concentrate certain important cells and active healing elements in form of biological soft membranes.

Sometimes, implant placement at sites with severe facial concavity cannot be achieved without fenestration.
In this case, Straumann BLT tapered implant was placed with some thread exposure on the facial aspect. Decision was made to perform contour augmentation at the time of implant placement with Puros cortico-cancellous allograft covered by Osseoguard membrane (both from Zimmer-Biomet). Periosteal tacking sutures with 4/0 Glycolon was used to stabilize the membrane. Flap was coronally advanced and primary closure with 5/0 Glycolon was accomplished. 4 months later the surgical re-entry showed excellent robust bone formation at the facial aspect, eliminating the concavity and achieving hard tissue coverage over the exposed implants threads.
Case after case I’m finding out that the results of these kind of procedure are very predictable due to consistent surgical techniques and the appropriate use of biomaterials based on the specific case-by-case situations.

Implant placement with contour augmentation to increase facial bone thickness and improve the slopped architecture of the facial bone. A 50:50 mixture of autogenous bone shavings from external oblique ridge were mixed with large particles allograft (Puros from Zimmer-Biomet). After decortication of recipient site, bone mixture was packed into the facial depression and covered with Osseouguard absorbable collagen membrane from Zimmer-Biomet. Membrane was stabilized with periosteal tacking sutures using 5/0 Glycolon sutures from osteogenics. Facial flap partially released and primary closure achieved using 5/0 Glyoclon.

Post-Extraction hard and soft tissue management. Concept from “A soft tissue pinch technique” by Hammele group
Atrumatic tooth extraction performed on tooth #8 due to non treatable endodontic infection. #15 blade use to harvest the tissue cap instead of biopsy punch. Socket grafter with cortico-cancellous Allograft (Puros) from ZimmerBiomet. The tissue graft was carefully sutured with 5/0 Glycolon to the de-epithelialized soft tissue margins of the socket. Multiple similar cases showed successful biological and esthetic integration of the transplanted graft into the recipient site tissue.

Bone grafting at the time of implant placement in order to increase the thickness of facial bone hoping to improve long-term stability of the final outcome. Autogenous bone particles mixed with Puros cortico-cancellous allograft from ZimmerBiomet. Osseouguard resorbable collagen membrane was stabilized with periosteal tacking sutures using 4/0 Glycolon. Soft tissue adaptation and primary closure using combination of internal horizontal mattress and simple interrupted sutures with 5/0 Glycolon.
