Pete Butkus DDS " I’m getting to the point in my biomimetic journey where I get to share follow-ups like this.

July 5, 2022 → December 31, 2025. — 3.5 years.

Tooth 5 came in with a fractured palatal cusp — mostly virgin tooth, a beautiful Class V from a mentor who referred her, and a situation where rushing to an implant felt like the wrong answer. So we didn’t.

The patient (Karen, she signed a release for this post) knowingly invested crown-level fees for a direct restoration. She understood the risk. She trusted the process. And at the end of 2025, she came back in — no symptoms, healthy tooth, zero regrets. You’ll see her say exactly that in the video. The patient matters more than any clinical photo I can take.

The clinical challenge here was matrixing a subgingival palatal margin with nothing commercially available that fit the situation. So I made a custom greater curve-style copper band, secured within a Tofflemeier retainer, and compressed it apically to achieve a seal at — and slightly below — the attachment level. Back in 2022, I wasn’t seeing this setup much. Now I see it more. That makes me happy.

This is also a reminder of why consistent, high-quality documentation matters. These photos — shot years apart — tell a story that a single appointment never could.

The last slide shows Karen and I in a video, sharing her perspective. Because zooming out from the clinical details and hearing it from the patient is the whole point.

“I know I have a full mouth of dentistry that needs to be redone… BUT can you save this tooth?” 😄

And that’s the moment you realize they’re hoping you’ll perform a little magic. 🪄

In this case, it was tooth #14.

The real magic, though, was the fact that the pulp still tested vital despite everything it had been through.

My job was simple: preserve as much healthy tooth structure as possible, establish a reliable Peripheral Seal Zone, maximize bond strength, minimize stress, and give that pulp every evidence-based chance to survive.

Then I go home… say a little prayer… and trust that the magic of evidence-based dentistry works its magic once again. 🤍

Changing the pich valves is easy!
