Congenitally missing lateral incisors generally require a team approach between the patient’s restorative doctor, the orthodontist, and the oral surgeon.
Cases like this come down to planning and collaboration. The orthodontist sets up the space, we place the implant, and the restorative doctor completes the restoration. This one was done fully guided for precision, using a surgical guide we designed and printed in-house. We also used a connective tissue graft to build up the facial gum tissue, giving the restoring dentist a generous, natural base to shape the final tooth.

Not every restoration journey starts with us, but we’re always grateful for the opportunity to help patients find a solution when challenges arise.

This case is a great example of how careful treatment planning and a customized approach can make a meaningful difference in the outcome. By reassessing the existing situation and developing a new strategy, our team was able to create a more stable and functional result for the patient.

This patient presented with a transverse discrepancy: an upper jaw too narrow for the lower, which crowds teeth and throws off the bite. They were referred to us by the team at in Waco, who did an excellent job pinning down the diagnosis before the patient ever reached our chair.

The solution was Surgically Assisted Rapid Palatal Expansion (SARPE) — pairing a precise surgical release with a palatal expander to widen the maxilla. We achieved a full ~8mm of expansion, opening up the arch width needed for a stable, functional bite.

The best part: because this was a pure transverse problem with no underlying maxillary hypoplasia, this patient is unlikely to ever need larger corrective jaw surgery (a Le Fort I) down the line. One targeted procedure, lasting payoff.
