Acrylic “temporary” Restorations placed as a prototype while we phase through this complex treatment for our sweet patient.

We were a bit unsure what surprises might lurk beneath older metal core crowns, but knew- no matter what- our patient could walk away with a smile that would begin a pathway to health. We removed more decay than anticipated, removed two teeth and placed graft material in the sockets.
These provisional crowns will serve as a test for all the criteria that needs to pass before transitioning into ceramic dentistry- esthetic appearance, function, stability of the underlying tooth, phonetics.

Dr. Heymann served as the Chair of the Operative Dentistry Department in Chapel Hill, serves as the Editor-in-Chief of the Journal of Esthetic and Restorative Dentistry, and is the Senior Editor of The Art and Science of Operative Dentistry- the #1 selling dental textbook of all time. He’s lectured all over the world thousands of times. He is my lifelong favorite teacher- and more than anything, he’s a great man and a great friend.

There are lots of things dentists do to feel validated in their career, be it an accreditation or some other sort of diploma, fellowship, or other merit badge to hang on the wall. For me, there was no greater honor in my career than to get a call this summer from Harald to treat a broken front tooth for him. Just a smalltown boy living in a lonely world- this was a really big deal!

Upper ceramics immediately after first try- in. This is a tough case-
It all started with a suspicion that decay was under a few decades old crowns. The older metal technology made the extent of damage impossible to see in radiographs, but it was obvious that attempts by the previous dentist at patching edges was not sufficient. Every tooth had a suspicious existing crown, every single one compromised. There was a difficult but honest conversation that the restorations the patient had enjoyed for decades would need to be replaced- or perhaps reconsidered.
Our only goal from the onset was to remove the old crowns and disease- decay, infections, periodontal compromise- and all of the biological factors that were contributing. To just say- “here’s what we’re doing” would be foolish going in blind. So we planned to use temporary crowns as we diagnosed.
We found teeth that were decayed beyond repair, dead and accessed, in need of structural buildup, and a bite that had wholly changed from ideal. The time in the temporaries was a time for healing and discovery. To be intentional to solve gum health- to prove that some of the pulps were going to be healthy.
This is an after of the upper ceramics. We are excited to place the lower final ceramics in about a month. So many times these posts seem to be about - “hey look at how this looks”. (And it looks like an overnight “extreme makeover” without telling the process.) The real celebration in a case like this is “hey look at the journey”.

So the perimeters- really dark damaged tooth structure (restorations will have to be somewhat opaque), high smile line, want to only restore four teeth and the neighbors are translucent and darker.
This is a tough one- we tried to blend/fade the darker chromatic areas as we came inward toward the midline. In order the have lighter teeth- it meant brighter teeth.
