Not All Gum Problems Are Caused by "Bad Hygiene"

When people think about gum disease they usually think of plaque, tartar buildup and bleeding gums caused by bacteria. And while that's often true... sometimes the real problem is something completely different.

This case is a perfect example. The tooth in this photo had a crown placed that was too bulky and over contoured near the gum line. Biologically, it created a major problem: trapped bacteria and food, difficult/impossible to clean properly, gums stayed chronically inflamed, deep persistent periodontal pockets developed, and ensuing bone loss.

In this case, the issue wasn't simply bacteria alone - it was the anatomy of the restoration creating an environment where inflammation thrived.

As periodontists, we don't just treat gums; we investigate why the gums are breaking down in the first place. Healthy dentistry isn't about how teeth look. It's about creating a design that the gums and bone can live long-term success.

"Dental implants last forever"... until biology decides to humble everyone. Even well place implants can develop bone loss over time. The difference is recognizing the problem early, and knowing how to rebuild what was lost.

Significant bone loss presented on this implant. Instead of removing the implant, it was treated with a regenerative therapy aimed at rebuilding support and preserving the implant long term. 2 years later we have bone fill, improved stability and healthier prognosis. Implants are incredible, but they're not maintenance-free. Sometimes the best dentistry isn't placing the implant... it's saving one.

Last week’s freeze / ice storm reminded me of a trauma case we recently completed here at Frisco Periodontics. Our patient slipped and fell face-first, fracturing his front tooth. In recognition of Veteran's Day, I am reminded about a special patient of mine. He presented with bleeding and painful gums due to recent crown placement too far under the gum tissue/too close to the bone. I was able to perform a painless gum flap procedure to fix the problem and save his teeth.

At the post-op check up his wife brought some photos of him in Vietnam circa 1969. Attached are photos of him standing on "Agent Orange Mountain," a carcinogenic herbicide and defoliant. He loved showing me the photos of him performing dental treatment on children in a nearby village.

These photos have a special meaning, of all the sacrifices (past, present, and future) these brave men and women have given for the freedoms we enjoy today. Never Forget. To this, we salute you!

People always ask: "What is a Periodontist?" Simplest way to answer is that they maintain the health of tooth supporting structures (gums and bone) beyond the capacity of a general dentist. What a difference a year makes! Dr Carnow sharing Halloween celebrations with his oldest son, Luke. The two dressed up as Blues Clues last year . This year Luke wanted to be Marshall, the Firefighter from Paw Patrol. He loved the festivities. Most importantly, extra brushing tonight to keep the bugs away!
