"Dental implants last forever"... until biology decides to humble everyone. Even well place implants can develop bone loss over time. The difference is recognizing the problem early, and knowing how to rebuild what was lost.

Significant bone loss presented on this implant. Instead of removing the implant, it was treated with a regenerative therapy aimed at rebuilding support and preserving the implant long term. 2 years later we have bone fill, improved stability and healthier prognosis. Implants are incredible, but they're not maintenance-free. Sometimes the best dentistry isn't placing the implant... it's saving one.

Last week’s freeze / ice storm reminded me of a trauma case we recently completed here at Frisco Periodontics. Our patient slipped and fell face-first, fracturing his front tooth. In recognition of Veteran's Day, I am reminded about a special patient of mine. He presented with bleeding and painful gums due to recent crown placement too far under the gum tissue/too close to the bone. I was able to perform a painless gum flap procedure to fix the problem and save his teeth.

At the post-op check up his wife brought some photos of him in Vietnam circa 1969. Attached are photos of him standing on "Agent Orange Mountain," a carcinogenic herbicide and defoliant. He loved showing me the photos of him performing dental treatment on children in a nearby village.

These photos have a special meaning, of all the sacrifices (past, present, and future) these brave men and women have given for the freedoms we enjoy today. Never Forget. To this, we salute you!

Another case of guided tissue regeneration utilizing mineralized bone allograft and resorbable barrier collagen membrane. Patient presented from general dentist with history of prophylaxis and lack of continual professional maintenance. Attempts were made to refer by gp but patient didn’t see a need. Comprehensive perio exam found generalized deep probings in the posterior 4 quadrants. I recommended a phased approach, which he declined. 4 weeks later he came back with a perio abscess on tooth #2 URQ second molar. Pain to biting, puss, bleeding. Vital pulp. Facial recession and thin biotype shown on this patient’s mandibular anterior central incisors. Lack of keratinized tissue and aberrant labial frenum further compound the chief concern. History of recent orthodontics and retained lingual wire. Sensitivity to brushing make it difficult to maintain proper hygiene. Connective tissue graft performed. 1 year post surgery shows what a difference thickening marginal tissues and covering roots can do. I love watching gums (and blood vessels) grow! Does “stippling” occur in the presence of inflammation? Stippling can be seen as small, alternating elevations and depressions on the surface
of the attached gingiva. The appearance is similar to that of an orange peel. Dr Balint Orban (1948) suggested disappearance
of this stippling could be one of the earliest signs of disease. In a 1962 study, Dr. Alan Greene found only 40% of patients to have
stippling. He concluded when gingiva was restored to health, stippling became evident. Take a look at the before and after pictures to view the reappearance of stippling once gingival health is restored!

A follow-up post from a previous case. Treatment of facial recession can be approached many ways, but autogenous connective tissue grafts are the accepted gold standard. Failures, or complications, may be fewer when harvesting from the palate (with the proper training). Miller class 1 facial recession and thermal sensitivity, treated conservatively with a tunneling technique. 12 week post op shows stable marginal tissues supported by an underlying band of connective tissue. @ Frisco, Texas
