Elevated Community Health We proudly offer healthcare services to the medically underserved, uninsured, or underinsured at affordable costs.

Elevated Community Health is a Federally Qualified Health Center (FQHC) that provides a full range of integrated care services inclusive of medical, dental, and behavioral health services through a team-based model of care. Summit Community Care Clinic is a Federally Qualified Health Center (FQHC) that provides a full range of integrated care services inclusive of medical, dental and behavioral health services through a team-based model of care. We also accept Medicaid, Medicare and private insurance. The Care Clinic works with patients to make sure their health needs are met first and foremost. The Care Clinic knows that healthy people are able to stay in work, provide for their families, and contribute to our local community. Service fees for all patients without insurance are based on income and family size. Patients without insurance will be charged according to a sliding fee scale—based on Federal Poverty Guidelines—if deemed income eligible. No patient will be denied service for their inability to pay.

What’s scarier than Halloween? Trying to fit one more appointment into an already packed family schedule. School-Based Health Centers can help make healthcare easier to fit into the day by bringing care closer to where students already are. Less time spent coordinating transportation and schedules. More time focused on helping kids stay healthy and ready to learn.

🎀 October is Breast Cancer Awareness Month.

Awareness is important, but knowing what to do next matters too. Take time to know your body, understand your family health history, and talk with your healthcare provider about your breast health.

Screening recommendations are different for everyone. Your provider can help you understand when screening may be right for you based on your age, health history, and individual risk.

This October, turn awareness into action by making breast health part of the conversation. 💗

Thank you, Summit Rotary Club, for supporting accessible diabetes care in our community! Elevated Community Health is grateful to receive $2,000 to support our Continuous Glucose Monitoring (CGM) Program. This funding will help ECH purchase 75 CGM sensors for low-income and uninsured patients living with diabetes. CGMs provide real-time blood glucose readings, giving patients an important tool to better manage their diabetes and prevent serious complications. We are grateful for Summit County Rotary Club’s continued partnership and investment in the health of our community!

Looking for a simpler way to plan for your healthcare costs? Elevated Essentials is designed to help make care more accessible for uninsured individuals and families, as well as businesses looking for healthcare options for their employees.

Choose the services that fit your needs, including primary care, dental, and behavioral health, with clear, prepaid plan options designed to make costs easier to understand.

Our Care Card program and Medicaid are still here. Elevated Essentials does not replace these programs. Our eligibility team will continue to help patients explore their options and ensure those who qualify are enrolled in Medicaid or our sliding fee discount program, the Care Card.

Our goal is to help every patient find the most appropriate and affordable care option available to them.
