Grossly decayed tooth—radiographically, it appeared non-restorable. After thorough caries removal, endodontic treatment, and a new post-and-core restoration, the tooth is now ready for a full-coverage crown.

I had the privilege of speaking with the AEGD residents at VA Long Beach today, just four weeks before they complete their program. We had a great discussion on practice management, associationship opportunities, and some of the key things to look for as they begin the next chapter of their careers.

Of course, I couldn’t let them leave without talking about one of my favorite subjects—Endodontics!

Congratulations to these outstanding doctors. They have already demonstrated excellence by graduating near the top of their classes from UCSF, UCLA, Michigan, and Western University College of Dental Medicine. Now, they are finishing the VA Long Beach AEGD program with the confidence, clinical skills, and determination to become highly competent dentists serving their communities.

6-month and 1-year follow ups on this Vital Pulp Therapy case ✨🦷

Mom is so thankful she found , which led her to a full team approach with restorative care by and endodontic care by to take care of her baby girl. 🤍

Before finding us, she had been told by two providers to either extract the tooth or simply “cut down the extra cusp” without endodontic and orthodontic therapy.

Treatment completed in May 2025 and today, we’re celebrating 3 important wins:

✔️ Pulp tissues remain healthy
✔️ Root development is complete
✔️ Composite restoration still looks beautiful and functional

Cases like this remind us how preserving pulp vitality can support continued root maturation and long-term success.

66yo male presented in significant discomfort (8/10 on contact) just one day before leaving town for 2 weeks—timing couldn’t have been worse.

Tooth #19 diagnosed with Necrotic pulp / Symptomatic Apical Periodontitis, with a very inflamed PDL. Given the urgency and travel plans, we proceeded with a single-visit RCT.

Working length established, thorough debridement and irrigation completed, and obturation achieved same visit—patient left comfortable and ready to travel pain-free 🙌

Cases like this are a reminder of how impactful timely endodontic care can be.

Shoutout to for getting me files, , impressed with the flexibility and ability to maintain a very conservative shape. This tooth had a thin mesial root form, and I was able to use a 17/.02 file to conservatively prepare the mesial canals and achieve a clean, controlled obturation.

Emergency case 🚨

Patient presented with severe toothache for several days. The affected tooth: a bicuspid serving as the distal abutment of a 3-unit bridge — making preservation especially important.

🦷 Diagnosis: Necrotic pulp with Symptomatic Apical Periodontitis
🦷 Treatment: Single-visit root canal therapy using GentleWave disinfection
🦷 Approach: Conservative access to preserve the existing crown and maintain the bridge

Thorough disinfection + minimally invasive technique = saving both function and structure.

Eight years later… patient returned to our office for another tooth.

This patient returned for treatment on #30, and I was happy to see him back for a recall on #31 — a previously cracked tooth treated in 2018.

He recently experienced pain and completed a course of antibiotics, which temporarily relieved the symptoms.

Diagnosis #30: necrotic pulp with symptomatic apical periodontitis.

I completed single-visit root canal therapy using the GentleWave® CleanFlow procedure.

No internal or external leakage.
No recurrent decay.
The existing crown was preserved and conservatively repaired with packable composite.

Long-term follow-up matters. Conservative dentistry matters. Saving natural teeth always matters.
