We regret to inform you that this location will be permanently closing.
As a valued patient, we want to ensure you continue to receive the dental care you need. If you require a new provider, we recommend the following steps:
Continue Care with Dr. Paulson: Dr. Paulson will continue practicing at Smiles 4 Kids in Durango, CO, where he will be accepting new patients. If you wish to continue your dental care with him, please contact the Durango office to schedule an appointment.
Transfer of Records:
We will forward your x-rays and/or records to a new dentist upon request. An authorization form for record release will be provided as needed.
Finding a New Provider:
You may seek recommendations from friends and family or explore options through the New Mexico Medicaid Provider Portal. Your insurance provider can also assist you in locating an in-network dentist.
