Not every full-arch case fits the same mold.
She came in with cavities, gum disease, and fractured, failing teeth, and she wanted a fixed, predictable smile with no more cavities or root canals. Her lower jaw had unusually soft bone, which is usually the other way around. We were ready: we finished her upper All-on-X and placed a temporary implant-retained denture on the bottom while she heals. Part 2 coming soon.

Yes, you can be numb for your dental cleaning.

If cleanings hurt, you don’t have to just grin and bear it. Here’s your first step:

1️⃣ Tell your hygienist or dentist. Let them know cleanings are uncomfortable for you, and what hurts: your teeth, your gums, or both.

2️⃣ Ask about numbing options. Oraqix is a numbing gel that goes on your gums with no needle, and local anesthetic is also an option. These aren’t just for deep cleanings. They can help with regular cleanings too.

3️⃣ Ask about laughing gas. It’s a great option if you have a strong gag reflex or just feel anxious in the dental chair.

A comfortable cleaning can be life changing, especially if you’ve been avoiding the dentist because of past experiences.

Peptides vs. fluoride: what does the research actually say?

Peptide treatments like Curodont Repair are getting a lot of buzz as a “no-drill” fix for early weak spots in enamel. The idea is exciting, and early studies look promising, but most are small and short-term.

Fluoride has decades of research behind it. The American Dental Association’s evidence-based guidelines recommend fluoride varnish or gel, often paired with sealants, for early spots that haven’t become a cavity yet.

What about cavities that have already formed? Silver diamine fluoride (SDF), used since the late 1960s, has an evidence-based guideline from the American Academy of Pediatric Dentistry for stopping active cavities in kids. One heads-up: it permanently stains the decayed area black, so it’s usually not the first pick for small spots, especially on front teeth.

The bottom line: the more proven, peer-reviewed research favors fluoride. Peptides may become a great tool, but we’re not there yet.

My routine is pretty simple: vitamin C and SPF in the morning, retinol at night. Lately I’ve added one more step, PRF microneedling.

The AAID Dental Implant conference was in Nashville this year, what a cool place to visit. Great to connect with colleagues, and I also took some fantastic hands-on courses. Happy to be back home to the NH foliage though!

See how that frenum is pulling right on the gum tissue there? Every time you move your lip, it tugs on it, and over time that constant pulling can cause the gum to recede, exposing the root and making that area more sensitive and harder to keep clean. That’s exactly why in some cases, we recommend a frenectomy, to release that tension before it causes more damage. Quick procedure, big difference for your long-term gum health.

Sealants are great at preventing cavities — but they’re not invisible shields. Decay can still form underneath if early demineralization was missed at placement, or if the seal breaks down over time. Doesn’t mean skip the sealant — it means stay vigilant with checkups. And for grooves already showing early decay, SDF can be a smarter first move: it stops the cavity and keeps the tooth surface visible instead of covering it up.
