- Good visual examination with magnifying loupes and previous clinical experience are required to diagnose caries and to be able to remove the caries completely.
- Most occlusal caries won’t show on a bitewing x-ray unless they are very large
- Some caries may look like a stain.
- Caries should be removed when it is still small, and should not be watching to grow larger under the dentin
- Don’t leave caries behind because you want to save more tooth structure. Removing a little bit more, it will not “kill” the tooth. Leaving it, it’s worse.
The tooth was restored with Estelite Sigma Quick composite.

Another case of larger soft caries growing under the hard pits and grooves.
In my opinion, it takes a little of bit of past experience to recognize the caries and to know what to drill and not to drill.
The tooth was restored with Estelite Sigma Quick composite.

Too many dentists ignore caries or stop removing it completely thinking that it is just a stain because they want to preserve the tooth structure, even though the caries/stain is very shallow, not deep into the dentin. In my opinion, the cavity preparation should always be clean and neat without any shallow stains/caries that could be a larger caries growing underneath. Not all caries are soft. Caries should be removed when it is still small with a conservative cavity preparation.
