Regenerative Endodontics. 14 years old patient. #20 PN/CAA. Immature apex

Regen Protocol

First visit: Access, Laser disinfection with diluted NaOCl. CH placed for two weeks.

Second visit: Sinus tract disappeared and patient is asymptomatic. Anesthetic with Carbociane to help provoke bleeding. CH removed, ran the laser with water to provoke bleeding instead of over instructing with a large hand file. Collagen matrix and BC Putty packed against it. Dual cure resin to close up.

One year follow up: very nice healing can be seen on both 2D & 3D images. Root closure can be seen as well. No discoloration was noted at the one year follow up and patient is asymptomatic.

I even asked the patient whether they recall an AutoTransplantation being done in the past, I’m thinking this looks like a premolar. But no Hx of that reported.

Tooth 🦷 Makeover 💫

#19 PT/CAA. Large distal Caries. Buccal sinus tract.

Multiple teeth isolation. Crown sectioned and removed. Caries and old build cleaned out, slight Gingivectomy done, and a new build up was placed. Switched to single tooth isolation and re-accessed.

Straight forward retreatment from there, laser disinfection and resin closure. Single visit treatment.

Two cases from a couple of weeks ago. Did those back to back. Class I & II resorptions lesions are treatable with decent prognosis.

#27 SIP/NAT & #5 SIP/SAP. Endos and cores done first, then flap raised and defect repaired with RMGI. Love doing those cases and I’m definitely doing more and more.

Surgery from this morning. #9 PT/CAA. Class II resorption defect can be seen on the CBCT on the palatal side. I decided to go full surgical vs retreat.

Buccal flap was raised first to treat the lesion apically. Resection, retroprep and retrofill with BC Putty. Allograft and PTFE sutures.

Then I made a small sulcular flap on the palate to treat and repair the resorption defect with Resin Modified Glass Ionomer. Polished very well after that. Sling suture to close up. Postop CBCT was taken for quality assessment. Recall is in order.

External Cervical Resorption Class II Management.

#4 incidental finding by the RD. Patient is asymptomatic. Small portal of entry below the crestal bone is detected on the scan. I decided to go with an internal repair approach.

Straight forward endo. Laser assisted irrigation and defect cleaning. Dual cure resin was bonded to a fiber post. I recommended holding off on the crown pending follow up, would you green light a crown immediately in those cases?
