This 15-year-old came to us with a few complex challenges—blocked-out upper canines, a class II bite, missing and unsalvageable lower first molars, and a midline that was noticeably off.

We created a treatment plan that balanced efficiency, function, and long-term aesthetics. By extracting the upper first premolars and the lower 6s, we were able to guide the canines into their correct position, shift the midline, correct the bite, and close all spaces—all without the need for future prosthetics.

The best part? His confidence grew alongside his new smile.

When creativity meets complexity. 🤩✨
This remarkable case challenged every textbook rule—Class III skeletal pattern, mandibular asymmetry, and a history of trauma to UR1 and U2-2 that left us with ankylosis, resorption, and an anterior open bite.

After failed attempts with TADs, vertical elastics, and even a subluxation procedure, we took a creative route: bracket repositioning to build a U2-2 step, followed by a multisegment Le Fort I with anterior downgraft and BSSO setback with yaw correction.

The result? A functional occlusion, improved bone position, and space for a future implant at UR1 that will finally do its job—contributing to long-term function and esthetics.

Presenting my very first case—and one of my all-time favorite patients!
She came in with a challenging skeletal Class III pattern, a retrognathic and narrow maxilla, full upper molar agenesis, lower second molar agenesis, severe upper spacing and rotations, dental Class III, and significant lower crowding with recession on LR1.

We achieved a stable Class I dental result by distalizing the lower arch using two Temporary Anchorage Devices (TADs). For retention, we used a Hawley retainer with pontic teeth to maintain esthetics and space until she’s ready for implants, along with a fixed upper bonded retainer. A laser frenectomy was also performed to help maintain space closure.

This case is a reminder of the importance of interdisciplinary collaboration—especially when restoring function and esthetics in patients who might ideally benefit from orthognathic surgery. Sometimes, with innovation and teamwork, we can push boundaries and still deliver life-changing results.

So proud of her transformation and honored to be part of her smile journey!
