We’re excited to announce something we’ve been wanting to do for a while at Cortez Family Dentistry!
We want to give one deserving person in our community a FREE smile makeover.
We’re looking for someone who is always giving to others. Maybe they volunteer, coach, teach, care for others, help their neighbors, or are simply the person who is always there when someone needs them. We want to do something special for someone who spends so much of their time doing things for everyone else.
Click on the nomination form below and tell us who you think deserves a new smile and why. After reviewing the nominations, we’ll invite selected nominees to Cortez Family Dentistry for a complimentary exam and X-rays so we can learn more about their dental needs.
From there, we’ll choose one person to receive their smile makeover completely free of charge.

Smile Makeover Rules:
1. You cannot nominate yourself.
2. Nominees must be 18 or older.
3. There is no cost to nominate someone or participate.
4. Nominees should be someone who gives back and makes our community better.
5. Selected nominees may be invited for a complimentary exam and X-rays.
6. Being nominated or receiving an exam does not guarantee selection.
7. One recipient will be selected for the complimentary smile makeover.
8. Treatment will be based on the recipient’s dental needs and what is clinically appropriate.
9. The smile makeover has no cash value and cannot be transferred.
10. Selection will not be based on insurance status, gender, race, religion, sexual orientation, disability, or other discriminatory factors.
