Most people think sleep apnea only means snoring or “stopping breathing.”

But many signs show up before someone ever gets diagnosed.

Here are a few I pay attention to:

• Tongue scalloping
Indentations on the sides of the tongue can be a sign the tongue is pressing against the teeth — often linked to low tongue space, clenching, or airway restriction.

• Loud snoring
This is not normal. It usually means airflow is being obstructed during sleep.

• Frequent urination at night
Waking up multiple times to p*e can be connected to sleep apnea because repeated breathing disruptions affect stress hormones and fluid regulation.

• Morning headaches
Often caused by poor oxygen levels, clenching, or fragmented sleep.

Here’s what most people miss:

You can have sleep apnea and never realize it.

Many people think they’re “sleeping fine” because they’re in bed for 8 hours — while their body is waking up repeatedly all night.

If you have these signs, it may be worth getting evaluated.

Your tongue is either expanding your jaw or shrinking it. Every single day.

Research in the Journal of Oral Rehabilitation and work by Dr. Sandra Kahn (Jaws: The Story of a Hidden Epidemic) confirm that resting tongue posture is a primary functional force shaping the upper arch.

The tongue exerts approximately 500g of force against the palate during proper posture — more than enough to influence bone remodeling during growth.

Correct tongue posture:
→ Tip just behind upper front teeth (not touching them)
→ Entire tongue body resting on the roof
→ Lips together, teeth slightly apart
→ Breathing through the nose

This is something you can practice right now.

Research in the American Journal of Orthodontics and Dentofacial Orthopedics shows that chronic mouth breathing leads to measurable changes in craniofacial development:

longer face height, narrower arches, and increased risk of sleep-disordered breathing.

Nasal breathing promotes proper maxillary (upper jaw) development through:

Early in my career, I focused on what I could see: cavities, broken teeth, gum inflammation.

Fix the problem. Move on.

But over time, I started noticing something I couldn’t ignore:

The same patients kept coming back… with the same issues.

That’s when I realized — I wasn’t treating the cause. I was treating the result.

Here’s what changed:

• I no longer ignore mouth breathing
It dries saliva, increases cavity risk, and affects jaw development — in both kids and adults.

• I no longer treat cavities as just “holes”
They’re a bacterial disease. If you don’t change the environment, they come back.

• I no longer tell patients snoring is normal
It’s often a sign your airway is struggling at night.

• I no longer focus only on teeth
Breathing, sleep, muscle function, and posture all matter.

• I no longer rush into braces without asking why
Crowding often points to underdeveloped jaws.

• I no longer overlook grinding and clenching
The body may be trying to keep the airway open.

• I no longer rely on mouthwash as a solution
It’s about balance, not eliminating all bacteria.

• I no longer ignore dry mouth
Saliva is your first defense against disease.

• I no longer do dentistry without thinking long-term
Every treatment should support function and longevity.

• I no longer treat symptoms without asking why
If you don’t fix the cause, the cycle continues.

This is when dentistry became more than teeth.

It became about:
• Airway
• Sleep
• Function
• Long-term health

Because once you fix the cause… everything else gets easier.

But here’s the part most people don’t realize:

If your jaw pain, tension, or headaches keep coming back…
it’s usually not just a muscle problem.
