Let’s look at this one in reverse.
1. Repaired tooth with DOBL Onlay. Milled emax. No, this one isn’t going to win any beauty contests, but with enamel-like properties and great bonding protocols, it will keep this second molar in the patients head and functioning like a champ
2. Fiber reinforced buildup with ribbond. The bonded ceramic may be enough, but in this case I wanted extra protection for the deep fractures.
3. End of preparation. All decay is removed and fractures are reduced (but not entirely eliminated) in this example. Blue arrows indicate residual dentin fractures. I should add, I am very confident the mesial area is in good structural condition. Although it may look like there is a crack there, this is likely a craze line only. I did not observe any dentin fractures and it looked very innocent on transillumination as well
4. Removal of old restoration. Lazy doctor using the low quality camera 🥴 😅. Blue arrows indicate several deep fractures threatening the comfort and longevity of this tooth.
