Let’s look at this one in reverse.
1. Repaired tooth with DOBL Onlay. Milled emax. No, this one isn’t going to win any beauty contests, but with enamel-like properties and great bonding protocols, it will keep this second molar in the patients head and functioning like a champ
2. Fiber reinforced buildup with ribbond. The bonded ceramic may be enough, but in this case I wanted extra protection for the deep fractures.
3. End of preparation. All decay is removed and fractures are reduced (but not entirely eliminated) in this example. Blue arrows indicate residual dentin fractures. I should add, I am very confident the mesial area is in good structural condition. Although it may look like there is a crack there, this is likely a craze line only. I did not observe any dentin fractures and it looked very innocent on transillumination as well
4. Removal of old restoration. Lazy doctor using the low quality camera 🥴 😅. Blue arrows indicate several deep fractures threatening the comfort and longevity of this tooth.

Nice case on an amazing teenage patient post-orthodontics. Strict protocols and attention to detail as always. These types of techniques are PARTICULARLY beneficial when helping young patients - 1. large and deep defects on younger folks can easily lead to complications like 1) need for root canal or 2) structural failures due to the increased use over time in repaired state (vs a healthy un-repaired tooth). If our strategy is corner-cutting, the frequency of restoration replacement needs will be high and this tooth will not last very long. The closer we can keep a repaired tooth to its natural, disease-free state, the better off we will be on our goal to keep teeth for life!

An example of a basic everyday case. Cold-sensitive tooth with leaking and cracking old and fairly deep (gingivally and pulpally) metal restoration. Transformed in a blue oasis. Not sensitive anymore.

Even well-done modern dentistry can fail. What’s important is HOW therapies fail, when we catch it, and what it costs a patient. This is a great example of a failure caught early. This restoration was replaced at minimal cost - with regard to finances and tooth structure. Because we caught it early, there was no time for decay or cracks to form. This contrasts dated dental therapy methods that often result in more damaging failure modes - resulting in more lost tooth structure, higher repair costs, and higher chances of losing a tooth.

Another example of an asymptomatic cracked tooth. As much as I would’ve liked to see a tooth with a relatively small existing restoration like this get another direct (filling) solution, the dentin cracks were too deep for me to feel comfortable not covering the cusps. Patient and I discussed and we ended up going with the most conservative full coverage option we could do. Decent ceramic integration - single visit dentistry

Teeth with large fillings, especially metal fillings that aren’t bonded, often progress to cusp fractures - as seen with this tooth. In fact, the broken cusp often isn’t the only part of the tooth that is damaged by fractures. In this case we had some deeper interproximal decay to address as well.

Teeth like this one are often treated with traditional full coverage crowns. Modern adhesive approaches allow us to do full occlusal coverage “crowns” that similarly protect the tooth but are much less invasive - meaning they require less total tooth structure removal. This is often referred to as a “biomimetic approach.” The aims of this approach are to use nature as a model to restore teeth. We remove only enough tooth structure for crown thickness and to address diseased tissue. The result is more preserved enamel and more dentin thickness insulating the tooth nerve. This means less post operative complications like sensitivity or root canal treatment. It also means both the tooth and the restoration can be stronger, and we can extend the restorative life cycle of the tooth, all while maintaining a natural appearance.

This case was treated in a single appointment utilizing Cerec technology with the restoration manufactured in house.
