The happiest of summers from Wyoming, Arizona, Tennessee, North Carolina and South Chesterfield! We took some days off but are back to doing what we love-saving teeth!! 🦷

Pretty bread and butter endo but had a minute to post yesterday's first and last cases, very different presentations and treatments, both great results:

First patient was having pain around a previously treated tooth--three canals had been filled in initial treatment (CBCT showed untreated second MB canal). There was a post in the palatal canal (well centered, and thankfully, metal!) and three plastic Thermofill carriers (not my favorite, but these came out fairly easily) to be removed. Previously treated canals were instrumented with Avalon BlueShaper Pros. The untreated MB2 was its own canal with its own exit (as seen on CBCT) and took a bit of effort to get to patency (D-Finders and Avalon SlimShaperPros for this one). GentleWave cycle was run, and buildup was placed.

Last patient of the day was referred due to deep caries. All endodontic testing was normal. Patient is in his early 20s. We discussed the range from direct resin to full root canal (with vital pulp therapy as an option, as well). Caries were fully removed, and no exposure was present. A direct resin was placed, and patient will be recalled in one year (or sooner if symptoms arise) to re-evaluate. I love doing root canals, but even more important is to do the best treatment for the given situation.

(It is painful obvious how bad my cassette-tape generation self is at making "pretty" posts, but the point is there!)

We’ll be closed today, Friday 6/5 through Sunday 6/7 to cheer onto Omaha.

…jk we’re always closed Fridays through Sunday, but Dr. Harmon sends all her long-standing Swayze crazy love to the Plains this weekend. Kiddos had a great time getting to watch a Rebel win earlier this season, and our Elliott got to meet Hunter Elliott who pitched his way to Omaha just after she was born in 2022. Here’s hoping for another! …added a throwback from the days before we had cameras in our pockets all the time. Happy weekend everyone!

I see a TON of external cervical resorption (I call it the v**e shop of the mouth--it's popping up seemingly everywhere!...not to be confused with it being correlated with va**ng, which has not been suggested to my knowledge).

In my nearly ten years as a general dentist, I had no clue about ECR. I just assumed everything was good ol' internal or external, which were treated with RCT. Turns out those are internal INFLAMMATORY and external INFLAMMATORY resorptions and totally different beasts.

External CERVICAL is by far the most prevalent form of tooth resorption, and it's one that is not caused by the pulp...so treating the pulp (which isn't the problem) doesn't solve anything. I tell patients all the time, sometimes the pulp is a casualty of the gums-gone-wild (or the treatment thereof), but they're not the source. The pulp tries so hard to protect itself and typically lays down a dense wall between itself and the invading tissue.

This CBCT (pt was referred for #19 ECR, which is more apparent...images included) shows a great example of the distal canal resisting the invading ECR #18. #18 has a normal, asymptomatic vital pulp in this case.

Posting this to show how incredible the pulp is at resisting ECR and some visual evidence that the pulp is not the problem. Sometimes ECR treatment is recommended which includes RCT in order to access the ECR tissue or because of proximity of the ECR tissue to the pulp. RCT alone will never arrest an ECR situation.

Lastly, in this patient, none of the teeth are symptomatic. #18 is beyond the point of treating and is asymptomatic and otherwise healthy. Treatment plan is NO treatment. If it becomes symptomatic or gingival issues arise in the future, EXT might be recommended.

#19 is incredibly deep to the furcation and asymptomatic. I discussed with the patient that treatment could expedite her losing this tooth. As is, the tooth is asymptomatic, and there is no sign of infection. Luckily, ECR can arrest and even repair. We'll watch this one, too.

#20 has a very tiny early stage ECR. This tooth is planned for a new crown. Treatment is advised and does not involve RCT!

A VERY belated Happy Administrative Professionals Day to this incredible team. We celebrated last week but didn't publicly show our love yet. Day 2 of was a success. More great classes, got to meet with endo residents researching for our future (not pictured, but the future is so bright for endodontics!), received my diplomate pin (photo with the biggest support system Mark), and more great catching up with my wonderful coresidents. Day 3 is shaping up to be great also. Continually grateful for the scientists, clinicians, and educators who continue to improve what we can do for our patients!
