Dental trauma is not always obvious. A tooth may look intact after a fall or impact while the pulp, root, or neighboring structures have still been injured. After an impact to the mouth or jaw, a professional exam can rule out hidden damage.

The location and depth of a crack can change the treatment options. Some cracks are superficial, while deeper fractures may threaten the pulp or make the tooth impossible to restore. Accurate diagnosis matters before deciding what comes next.

Cracked-tooth pain can be unpredictable. One common clue is a sharp sensation when biting down or when releasing pressure, even if the tooth looks normal from the outside. Intermittent bite pain is worth evaluating.

Not every pulp-related problem is managed the same way. In carefully selected cases, vital pulp therapy focuses on maintaining healthy living tissue so the tooth can continue to develop and function. Treatment depends on the tooth, age, and diagnosis.

WHY ENDODONTISTS MUST ALSO BE CAPABLE RESTORATIVE DENTISTS:

This case presents a seemingly straight forward RCT through a PFM crown. However; understanding that this patient uses tooth #28 as an abutment for a distal extension RPD is enormously important when designing the access through the survey crown. The red arrows illustrate that the access is heavily shifted towards the mesial so that the distal rest seat is not compromised. This particular rest seat has a vaneer of porcelain, which could lead to the RPD no longer fitting at all if the porcelain fractures. Accordingly, it is vital to understand restorative principles that assert a minimum of 1 mm thickness of feldspathic porcelain is required over the coping and between margins to minimize the risk of shear strain leading to fracture. Above, you can see that the access is 1.5 mm away from the mesial marginal ridge and 1.0 mm from the mesial most surface of the rest seat (black arrows). Clearly, there is very little room for error in these situations which is why microscope aided access becomes a huge asset for treating these teeth.

CBCT Guided Selective Retreatment:
This case presents a retreatment of tooth #15. Based on the PA, the treatment selection seems straight forward: retreatment including post removal and possible crown replacement. However; CBCT shows that the post (which appears very well centered on PA) is actually quite heavily transported in the palatal direction nearly perforating the palatal surface. Removal of this post carries a high risk of inducing a root fracture due to how thin the dentin is at that point. Fortunately, CBCT also shows that the lesion is localized around the buccal root (MB/DB fused root). This allowed for selective retreatment of only the buccal canals which resolved this patient's pain and sinus tract while also allowing for an extremely conservative access that would typically be much larger to remove the large palatal post.
