- Most horizontal root fractures maintain pulp vitality and long-term survival when the coronal segment is stable
-CBCT is essential to confirm healing patterns and avoid overtreatment
-Treatment is indicated only if necrosis, infection, or instability develops

Clinical nuance:
Mid-root fracture combined with periodontal bone loss reduces structural reserve. Prognosis becomes dependent on occlusal load management and ongoing monitoring.
