Conservative Management of a Healed Root Fracture 🦷📚🦷✨

Patient referred for eval of possible fracture on #12.
CBCT findings show a healed horizontal mid-root fracture, likely from remote trauma. Tooth remains vital, stable, and free of periapical pathology. No endodontic treatment indicated.

Evidence-based rationale:
- Most horizontal root fractures maintain pulp vitality and long-term survival when the coronal segment is stable
-CBCT is essential to confirm healing patterns and avoid overtreatment
-Treatment is indicated only if necrosis, infection, or instability develops

Clinical nuance:
Mid-root fracture combined with periodontal bone loss reduces structural reserve. Prognosis becomes dependent on occlusal load management and ongoing monitoring.

Thursday cases like this remind us why second molars keep us humble 🦷✨

#31 PN/AAP The distal root had a 90° exit with an additional lateral canal—definitely not a straight-line case.

Root canal therapy was completed through the crown using GentleWave to address the complex canal system.
Four-month follow-up shows beautiful radiographic healing and resolution of symptoms.

Vital Pulp Therapy vs Root Canal in Teens: What the Evidence Really Says🦷✨

A deep caries in a 12-year-old’s molar doesn’t automatically mean a full root canal. Thanks to Vital Pulp Therapy with modern bioceramics, we can often save the nerve , strengthen the tooth long-term, and avoid the structural thinning that happens with early root canal therapy.

In young permanent molars, the evidence is clear: when symptoms are mild and the pulp is still healthy enough to control bleeding, VPT can deliver success rates comparable to root canal treatment while preserving natural dentin and keeping the tooth stronger for decades. 🌱 If VPT ever fails down the road, a root canal remains a reliable backup option.
At BCC Root Canal Specialists, every treatment plan starts with biology, long-term function, and the patient’s age in mind. For kids and teens, conservation matters. 💛

Have questions about deep caries in young patients? We’re here to help you choose the safest and most predictable path forward.
