Implants can be permanent unless infection from plaque destroys the bone and gums.
The implant threads and roughened surface, if covered in plaque, are very hard to clean so the risk for on-going infection and additional bone loss is high.

A Tissue Graft in this case locked the tissues down (now strong and attached).

Now 8 years later. Surgery by Dr van Kesteren

Dr van Kesteren here with a new implant topic and the main reason for implant failure, bone loss from infection called Peri-Implantitis.

ADA Code D6049. Scaling Implant with Peri-Implantitis (equivalent is periodontal disease infection and bone loss around teeth)
Removing all bacteria and actually detoxifying the roughened implant surface is a HUGE challenge. Bone loss very often is irreversible.
Early detection is critical.
Implants can be permanent but reasons for tooth failure can cause Implants failure.

Keys for Prevention:
1. Excellent Daily Oral Hygiene
2. Routine Dental cleanings for oversight

Challenges to cleaning around Implants:
1. Access!!! (Tissues in the way; cleaning under crown contours)
2. Not scratching up smooth implant parts. Traps more plaque!

Part 2. A Smile Transformed. Orthodontic extrusion brings down bone and gums previously lost most commonly by infection or trauma. Dr Selden put the patient in a great position for 2 implants with crowns for the most esthetic and natural outcome and SMILE. This is a tough case made simpler with good planning.
