In order to keep cost and fees at a minimum for all our patients, it is expected that fees are to be paid at the time services are rendered. If you are to undergo extensive treatment, we will be happy to work out financial arrangements with you. There is a finance charge of 18% APR on all accounts open after 90 days. Just as we recognize the value of your time, we know that you appreciate the value of the doctor’s time as well. To be fair to all our patients, there is a charge of $40.00 per hour scheduled for appointments broken without twenty-four (24) hours notice. For Patients with Dental Insurance
As a courtesy to our established patients, we will file insurance claims through our office. We will, however, expect payment for the percentage of the total bill not covered by insurance. To expedite processing, please complete your part of the form accurately. We cannot be responsible for negotiating the settlement of a disputed claim. The ultimate responsibility for payment rests with the patient. If an insurance payment has not been received within 90 days, the account must be paid in full. Payment Plans
