Another case that was originally slated for an apicoectomy… and if you know me, you know apicos are not my go-to.

Less than 1% of what we do at Evolution Endo + Perio falls into that category. I reserve them for teeth with large posts where removal risks fracture or losing the crown.

If we can retreat a tooth, that’s almost always our preferred and most predictable option.

This case wasn’t exactly predictable either: furcal bone loss, heavy calcification, and buccal swelling. In fact, I tried to talk the patient out of retreatment.

He didn’t budge… and he was right.

At the 6-month recall, the CBCT showed furcal bone remineralization, complete soft tissue healing, and zero symptoms.

The body wants to heal, and it usually will once the true source of infection is eliminated. In this case, that source was a completely uninstrumented MB2.

Go figure.

When one tooth shows up with two roots, two pulp chambers, and one crown, you know it’s going to be a fun day. My wife always says I like a challenge, and she’s right.

Maxillary anterior fusion isn’t common, but this one definitely kept things interesting. Cold testing gave just enough reaction to reveal a bit of partial necrosis hiding out.

Why do two when one will do the trick?

Good thing for CBCT, because that’s where the real story showed up root-end resorption and a canal exiting a couple millimeters short of the radiographic apex.

With minimally invasive techniques, the GentleWave System, and the world’s most awesome endodontic assistants, even the wild cases become smooth saves.

Sometimes the greatest impact happens outside the office.

For years, Dr. Yuki from has dedicated his time to coaching youth sports, showing up for practices, games, and families because he genuinely cares.
