“Why are you so obsessed with me?” - MB2, probably.

Jokes aside, what makes MB2 so important? Many studies have shown this little canal can make a big difference in treatment outcomes. One study we conducted at the Tufts University School of Dental Medicine Endo Department (https://pubmed.ncbi.nlm.nih.gov/33285204/) found that maxillary first molars with a successfully obturated MB2 had a significantly higher success rate than those without (~97% vs. ~91%). It’s worth the search.

In this case, the MB2 was pretty cooperative. It was easy to find and merged with the MB canal soon after. Short cameo, but we’re always happy when it makes an appearance. 🍿🦷

Honestly, I was not expecting to have to search quite so far for this little canal but it did make finding it more fun! 😄

Included here is a slice of the CBCT we captured right before the discovery, and the file shot immediately after, just to confirm we were where we needed to be. That’s always a happy moment!

Wishing everyone a week full of wide open canals! 🦷✨

What is endodontic microsurgery AKA an apicoectomy?

Most of the time, a root canal is all a tooth needs to heal and thrive. But occasionally, the area at the very tip of the root (the apex) struggles to fully heal.

Think of a tooth’s root like a tree: there’s a main trunk, but also a lot of tiny branches at the bottom. If one of those tiny branches is too small to clean through the main canal, or if a cyst just isn’t responding to conventional treatment, the problem can linger.

That’s where an apicoectomy comes in.

Through a small incision in the gums, we can access the tooth’s apex, remove the last few millimeters of root (where the problem lives), clean everything out, and seal it back up. It’s a straightforward outpatient procedure, and most people are back to their normal routine before they know it.

We consider it when: → A previous root canal hasn’t fully healed → There’s a persistent infection or cyst at the root tip → Retreating through the crown isn’t feasible or has an unfavorable prognosis (think: massive post) → We want to save a tooth that’s worth saving 🦷
