Nothing stops a restorative plan quite like a fractured screw. This patient came in with a broken abutment screw lodged deep inside the implant.
1. The Challenge: Accessing the fragment without harming the implant threads.
2. The Result: Total retrieval and one very relieved patient!
The implant is saved and ready for its new crown.

When I first saw this case in 2022, the odds were stacked against us. Multiple broken files and a large lesion (radiolucency) meant the tooth was on the brink of extraction.
Instead of giving up, we went to work. It took precision to navigate around those broken instruments, but bypassing them opened the door to healing.
Fast forward to 2026: The patient came in for a simple cleaning, and a quick check-up X-ray revealed beautiful bone healing. There is no better feeling in dentistry than seeing your work stand the test of time!

Interesting retreatment case with broken file in mb canal and huge lesion .
Gp was removed and file bypassed then file jumped apically due to wide apexes so decided to remove it .
Then a lot of irrigation and activation and CaOH between visits .
It took 3 months to obturate this case after getting dry with apical MTA then coronal with Thermoplastisized gutta percha hopefully for a second chance .
Waiting for follow up
