What's the difference between cosmetic dentistry and biomimetic esthetic dentistry?

In my opinion, a lot! 😁

Too often, cosmetic procedures lack the quality and precision needed to prevent complications. They focus on esthetics without addressing function. Teeth are heavily reduced, margins are inadequate, and cementation protocols are simplified for speed rather than longevity. This results in a higher risk of complications such as cavities, periodontal issues, and root canals.

This patient was seriously considering extractions and implant dentures after multiple cavities and failed restorations from previous cosmetic treatment. The work had been done only five years earlier, yet it was already failing rapidly. He was told most of his teeth were beyond saving and implant dentures would be the best option. Fortunately, with a biomimetic approach, we were able to save his teeth and prevent this outcome.

Not all cosmetic dentistry ends this way, but unfortunately, too much of it is performed under inadequate conditions. In this case, poor quality work accelerated complications, compounded by the patient’s own contributing factors such as diet and oral hygiene.

Biomimetic esthetic dentistry is different. It maximizes results in both function and esthetics. By restoring natural strength, biomechanics, and beauty without compromising long-term durability, this approach gave us the opportunity to save teeth in very poor condition and give them a second chance.

His full mouth was restored biomimetically. These cases are challenging, but the results are worth it. One of the greatest advantages of biomimetic dentistry is the ability to preserve teeth like this and extend their life. Ideally, treatment done properly from the start avoids these complications altogether and minimizes tooth loss.

This patient received 20 porcelain veneers 10 upper and 10 lower cemented one by one under rubber dam. This approach isn’t about convenience, it’s about quality and control. Cementation is one of the most critical steps in the treatment process. The rubber dam creates a clean, dry environment that maximizes adhesion. Each veneer margin is precisely sealed and seamlessly finished, helping prevent many of the common problems associated with veneers.

Cracks are a biomechanical problem. 🦷
Intact teeth distribute forces through a stiff outer enamel shell that limits tensile stress by converting functional loads into compression. This is a key reason natural teeth resist fracture so well.

Too often, when teeth are restored, the restoration fails to adequately restore stiffness and normal force distribution. This leads to unnatural stress concentrations, increased cuspal flexure, and ultimately crack propagation.

The ideal restorative approach is to restore the structural integrity of the tooth using the least invasive strategy possible.

Crowns can protect cracked teeth by redistributing stresses, but they require substantial additional tooth reduction. That added removal significantly weakens the remaining tooth structure and makes it increasingly difficult to restore normal biomechanics. While crowns can successfully protect cracked teeth, the associated loss of tooth structure increases the risk of long-term complications and ultimately tooth loss.

🎯 The goal for restoring any tooth is to restore biomechanics while preserving natural tooth structure.

In this case, two cracked teeth were restored using

• GC LiSi overlay restorations to preserve tooth structure and control stress concentrations

Preserving tooth structure through conservative treatment increases long-term success and reduces cumulative tooth loss over a patient’s lifetime.

✨ In the end, natural esthetics are always the most beautiful.

This patient never liked her smile, so she had a full mouth of veneers completed. Unfortunately, that backfired. She still disliked the result. The restorations appeared unnatural, with a bulky, opaque, heavy appearance that overpowered her face.

A smile needs to be in harmony and balance with the face to look beautiful and natural. While I can’t change a patient’s facial features, every smile design should be facially driven, with overall balance, beauty, and esthetics as the top priority.

For her treatment, we addressed the gum asymmetry and replaced the upper restorations, focusing on improved contours, shapes, and proportions for a natural result. She chose a natural shade (1M1) because she did not want her smile to look like veneers. That said, even whiter shades can look natural when they incorporate translucency and color gradients rather than appearing opaque and uniform.

More patients are moving away from a “Hollywood” smile and toward results that feel natural and personal. My goal is to review these options carefully and ensure each patient has a smile that reflects who they are. Being confident with your smile is what matters most. ❤️

She had been self-conscious about her smile for as long as she could remember.

She was born without her lateral incisors, so her canines were orthodontically substituted into those positions and composite bonding was used in an attempt to improve the appearance. The result was an imbalanced smile with size and color discrepancies, irregular embrasures, and uneven contours that left her feeling insecure about her smile.

With 6 ceramic veneers, we restored balance, proportion, and symmetry.
