Angular Cheilitis

🔹 Inflammation at lip commissures
🔹 Presents with erythema, cracking, fissuring, soreness
🔹 Often a mixed infection: Candida albicans + Staphylococcus aureus

Common causes / associations
🔹 Decreased vertical dimension (VDO) = classic board cause
🔹 Seen in edentulous patients or those with old/worn dentures
🔹 Overclosure causes saliva pooling at mouth corners
🔹 Also associated with HIV, uncontrolled diabetes
🔹 Iron, riboflavin (B2), folate, B12 deficiency
🔹 Mouth breathing, dry cracked lips, Down syndrome

Clinical clues
🔹 Painful fissured commissures
🔹 May have crusting, maceration, burning
🔹 Often recurrent if underlying cause is not corrected

Treatment
🔹 Topical antifungal: nystatin or clotrimazole
🔹 Topical antibacterial: mupirocin if bacterial component suspected
🔹 Mild topical steroid may help inflammation
🔹 Definitive treatment = correct cause
🔹 Restore proper VDO, improve denture fit, treat deficiencies/systemic disease

🔹 Edentulous patient + overclosed bite + fissures at mouth corners = angular cheilitis
🔹 Treating infection alone is not enough unless the underlying cause is fixed
