We regret to announce that Lakeway Dental Center will no longer be open for business after January 16, 2026.

An email copy of patient records will be available upon WRITTEN REQUEST through the office or by email until January 16, 2026.

After January 16, 2026, records may be requested by email only: [email protected]

Email MUST include the following:
1. Patient’s First and Last Name
2. Patient’s Date of Birth
3. If not self (minors), First and Last Name of person requesting records
4. Relationship to Patient
