Clefts of all types are often linked with congenitally missing teeth, and have been found to have a higher occurrence of dental anomalies, such as differences in tooth number and position, and reduced tooth sizes most of which are concentrated in the area of the cleft defect.
⠀⠀⠀⠀⠀⠀⠀⠀⠀
This patient was preparing to enter college and expressed to me his disinterest in having something removable that he would have to take in and out or something that would cause him to speak differently in front of his friends. Understandably, I was able to provide him with different fixed treatment options for his congenitally missing tooth such as space maintenance, restoring the space with a dental implant or resin bonded fixed partial denture, also known as a Maryland bridge which was the option that he preferred.

This case involved teamwork and collaboration with Dr. Adriana Da Silveira who is an orthodontist with additional expertise in caring for patients born with cleft lip & palate and craniofacial syndromes.

There are a number of people who have dental problems that must be treated comprehensively with a vision of a final result that improves both function and esthetics. These patients may be experiencing cracked or broken teeth, multiple missing teeth, numerous teeth with large fillings that are failing, or badly worn teeth due to teeth grinding or other habits.

The treatments can include veneers, onlays, bridges, implants, dentures and in this patient’s case, all crowns that will essentially provide not only a "smile makeover", but improved chewing efficiency and confidence for my patient.

🦷✨That back to back double cord technique 💜💛

♦️Keeping the margins free of fluids is key for an accurate impression. Hemostatic agents, mechanical retraction with cords, retraction paste, or a combination of any of these help to ensure that the margins remain visible and dry. Widely used hemostatic agents include:

🔸Aluminum chloride🔸
25% - ViscoStat Clear
21.3% - Hemodent
15% - Traxodent paste

It has been found that aluminum chloride is one of the least irritating among hemostatic agents used with cords.

♦️Ferric sulfate♦️
20% - ViscoStat
15.5% - Astringedent

Ferric sulfate stains gingival tissues due to its iron content and tends to leave behind a blue/black precipitate for several days. ⚠️ Care must be taken when using it in esthetic anterior cases.

🔸Both agents disrupt the setting reaction of PVS impression materials. Therefore, all traces of the medicament should be rinsed and cleaned off thoroughly from the tissue before taking an impression 🚿🧼
