Old bonding, new crowns 👑

After having had many rounds of bonding, this patients was ready for something permanent! He asked if we could improve on his natural smile, and enhance it. Our answer: no problem! 😊

The old bonding on the two front teeth didn’t match well, so we started this case like we start most “cosmetic enhancement” cases: whitening. Two weeks of our take home overnight whitening treatment brightened his natural teeth. We prepped 8 and 9 for crowns, and 7 and 10 for veneers. 🦷

By starting with the end in mind from our digital waxup, we were able to do ideal digital dentistry. We only removed a minimal amount of his natural tooth structure and old restorative material to make room for our new restorations. Photos were key in this process to ensure we matched the shade to what we wanted to achieve, and the master ceramist at Midwest Dental Arts went to work! These final restorations are pressed layered eMax.

Being a police officer can be a very stressful job! This patient had extreme wear on his bottom teeth and some on the upper arch. 😬

We underwent a full mouth rehabilitation to restore his aesthetics and function. We started by re-establishing his vertical dimension, or the height of his teeth. We then allowed him to “try out” that new bite for a few weeks to ensure his jaw joint didn’t become symptomatic. After a good prognosis, we began the process of crowns. The crowns were all created in our office using CEREC; we milled individual crowns for all his teeth and cemented them that same day! In one visit we worked on the upper jaw, then the following week, we completed the lower jaw. His new smile was his retirement gift to himself, and we were thrilled with the results! Thank you for your service 👮‍♂️

How exactly are implants structured? 🤔

Dental implants are medical devices surgically implanted into the jaw to restore a person's ability to chew or their appearance. They provide support for artificial teeth, such as crowns, bridges, or dentures. Dental implant systems consist of a dental implant body and dental implant abutment and may also include an abutment fixation screw. The dental implant body is surgically inserted in the jawbone in place of the tooth's root. The dental implant abutment is usually attached to the implant body by the abutment fixation screw and extends through gums into the mouth to support the attached artificial teeth. 🔩

This patient came to us with a severely decayed dentition looking for options to restore his confidence, smile, and function. 😁 After discussing many options, we needed to find a compromise somewhere. With a primary desire for fixed upper teeth, we opted to do an upper hybrid “All-On-4” and restore the lower arch with an overdenture consisting of two implants.

While this case is one of my favorite “before and afters” – it also highlights the main difference between using fixed and removable implant options. For overdentures, we typically put implants in different positions to build and contour the bone differently than we would for a fixed hybrid. The main difference is the posterior implants; overdentures usually get straight implants, and “All-On-4” traditional hybrids get angled posterior implants to improve the AP spread or the distance between the anterior and posterior implants. Thankfully, in this case, we could position the two implants in a space that would allow him to go with either fixed or removable options in the future. 🙌

Thankfully, after 3 months, we make a new overdenture that perfectly fits the gums and implants and snaps in and out! This is however the biggest difference between overdentures/snap in dentures and fixed hybrids — the healing period.

Periodontal disease is one of the many reasons why we reach for hybrids for our patients with remaining teeth. This patient came to us with mobility on every one of his remaining teeth, and almost no bone supporting his remaining dentition.

Because of his mobility, he had an unrepeatable bite. That made this case very difficult. Finding the correct bite for him took a few versions of temporaries. Due to the mobility and the location of his upper front teeth when we met him, we struggled to get his aesthetics perfect until his later temporaries. Thankfully, after a few versions; we were able to get something we all were happy with! 🙌

Thankfully, in addition to a lovely smile, getting to know this lovely patient was the highlight of this case.
