Appointment Preparation 🩺✅

Make the most of your health appointment by preparing in advance. A little organization can save time, reduce stress, and ensure your concerns are addressed.

If you are uninsured and utilizing our sliding fee scale please bring in your household income for the last 30 days. We can accept weekly or biweekly paystubs, 1040 tax forms, and benefit letters for SSI, Veterans, and Pension.

Both Medicaid and Sliding Fee Scales are renewed annually, you can always ask for your expiration dates to stay on top of renewals!

Our Financial Navigators are available at all of our health centers to help with insurance options, our Sliding Fee Schedule, food insecurity, housing resources, community resources, and other social determinants of health.
