It is a great question! The truth is, cavities do not always hurt right away. The outer layer of your tooth, the enamel, does not have any nerves. You can actually have a cavity growing there for months without feeling a single thing.

The intense, throbbing pain only starts when the decay travels deep enough to breach the inner pulp chamber where the nerves and blood vessels live.
