This patient presented with gross caries of 7 necessitating extraction, loss of vertical dimension from GERD-related erosion and bruxism, and financial limitations that made implant therapy less favorable.

Minimal restorative space present. A PFM cantilever from 8 was chosen because it allowed for conservative lingual reduction while still providing the esthetics needed in the smile zone. The facial margin was prepared as a 120° porcelain butt joint and maintained a metal lingual design. The preparation was designed as a traditional wing-style transition for added retentive and resistance features, and immediately sealed. The laboratory tribochemically treated the intaglio surface to enhance bonding to the metal substructure. Scotchbond Universal Plus with RelyX Ultimate were used for cementation. Utilizing every available source of retention, the facial porcelain was also bonded to maximize resistance to debonding.

The cantilever pontic was completely relieved from occlusion, and an ovate pontic design was developed immediately following extraction to improve emergence profile and tissue architecture. A Maryland-style wing extension onto #6 was not utilized due to the increased restorative complexity associated with path of draw and pin retention requirements. 🦷

Not every missing tooth needs an implant.

This patient presented with a single missing tooth but had a history of a heart attack and preferred to avoid implant surgery. The adjacent teeth were healthy, and he did not want to prepare them for crowns. His primary goals were to maintain function, preserve the position of the surrounding teeth, and keep the restoration as conservative as possible.

A unilateral removable partial denture was fabricated to replace the missing tooth. While these restorations are used infrequently due to the risk of dislodgement and aspiration, the design was modified by extending the framework to increase the footprint, improve retention, and enhance safety.

Sometimes the best treatment isn’t the most complex treatment—it’s the one that best fits the patient’s goals, health history, and comfort level.

This patient presented with a parulus on the buccal of #10. Clinical exam revealed a “shallow” lingual restoration with no pain or percussion sensitivity. The tooth was non-responsive to cold and EPT, with a developing periapical radiolucency consistent with pulpal necrosis.

After removal of the lingual restoration, pulpal exposure confirmed the source of contamination. Working length was established at 20.5 mm, a glide path was created to a size 20 hand file using Glyde, and shaping was completed with a Primary WaveOne file. The canal was obturated using a matched-taper single cone and Edge Bioceramic Sealer, finishing approximately 0.5 mm short of the radiographic apex. The access was permanently restored the same day.

I use the WaveOne system because it provides a predictable, efficient workflow with excellent compatibility between instruments and obturation materials. Consistency is one of the biggest contributors to successful endodontic outcomes.

Dentistry isn’t always about making teeth look perfect—it’s about making them look like they belong.

This patient was restored using a porcelain-fused-to-metal crown designed to blend with the adjacent teeth. Custom staining was applied to the cervical portion of the restoration to mimic the recession present on the neighboring teeth and create a more natural appearance.

The implant was placed by an amazing local oral surgeon who consistently does excellent work. Teamwork makes cases like this possible.
