When a previously treated tooth starts acting up again, the question on everyone's mind is the same. Can it still be saved? 🦷

The answer comes down to a few key factors we assess together, not in isolation. Is the root structure intact, or has it cracked or fractured? Can the canals be properly accessed and cleaned, even with their original anatomy? Is there enough healthy tooth structure left to support a crown long term? And is the infection localised, or has it spread more extensively into the surrounding bone?

A tooth that scores well across most of these factors is usually saveable, often through retreatment or a procedure called an apicoectomy. When several factors point against it, extraction may genuinely be the more reliable path forward.

Delaying the permanent crown leaves the tooth exposed to biting forces it is not built to handle alone. Grinding or clenching at night puts repeated stress on the tooth, sometimes leading to a crack years later. Biting on ice, hard sweets or using teeth to open packaging can undo treatment with a single hard bite. And skipping follow-up check-ups means small changes can go unnoticed until they become bigger problems.

Referred pain happens when discomfort is felt in one spot but actually originates elsewhere, and in the mouth this is more common than people realise. A neighbouring tooth with its own small issue, jaw joint problems from clenching or grinding, and even sinus pressure can all feel remarkably similar to the pain people associate with the treated tooth.

This is exactly why persistent pain after treatment deserves a proper reassessment rather than an assumption. Testing surrounding teeth, checking the jaw joint and considering sinus involvement all help identify exactly where the discomfort is truly coming from.

There is a lot more that happens before a root canal begins than most people realise. 🦷

It starts with listening closely to your symptoms, when the pain started, what triggers it, whether it lingers. These details often point us toward the source long before any instrument comes out.

From there we test how the tooth's nerve responds, take a detailed three-dimensional scan to spot cracks or infection that a regular X-ray simply cannot show, and examine the tooth under a surgical microscope magnifying up to 25 times.

Only once all of this comes together does a treatment plan get finalised. Nothing is decided on guesswork, and that groundwork is exactly why outcomes tend to be so predictable.

As endodontists there are a few specific things we look out for that genuinely concern us more than the average toothache. 🦷

Pain that suddenly stops can mean the nerve has died, not that the problem resolved. Swelling that spreads toward the jaw or eye signals infection moving into deeper facial tissue. Fever alongside tooth pain suggests the body is fighting something more significant. And difficulty swallowing or breathing is a medical emergency, not something to bring to a dental appointment.

Let's clear up five root canal myths we hear way too often. 🦷

"Root canals cause cancer." Myth. This claim traces back to a debunked early 1900s theory with no scientific basis behind it.

"You only need one if you're in pain." Myth. Pain is not a reliable indicator. Some infected teeth cause no pain at all until the damage is advanced.

"Root canals take multiple painful visits." Myth. Most are completed in one to two visits under local anaesthesia, with modern techniques making the process far faster than it used to be.

"You can leave an infected tooth alone if it doesn't hurt." Myth. No pain does not mean no infection. It can spread quietly regardless of how it feels.

"It's better to just extract the tooth." Myth. Saving your natural tooth is almost always the better long-term choice compared to extraction and replacement.
