🦷 Case Report β€” Pulpectomy of Primary First Molar ( #84)

Patient: 7-year-old child
Tooth: Lower right primary first molar ( #84)

Diagnosis:
β€’ Necrotic pulp
β€’ Chronic apical periodontitis
β€’ Periapical radiolucency with bone loss (more in mesial root)

Treatment:
β€’ Complete pulpectomy
β€’ Root canal obturation using Zinc oxide–eugenol (ZOE) paste
β€’ Coronal restoration completed
β€’ Follow-up scheduled at 3 and 6 months to monitor clinical and radiographic healing 🦷✨

Extraction of tooth #36 that had:
– Extensive deep caries reaching the furcation area
– Thinning of remaining dentin
– Severe structural loss
– Mild bone rarefaction from chronic inflammation
β†’ making the tooth non-restorable

Procedure:
– Surgical extraction with root sectioning, as shown in the intra-operative photo
– Deep curettage of the socket
– Irrigation with normal saline
– Pre-operative panoramic X-ray for assessment
– Post-operative panoramic X-ray confirming a clean site with no retained fragments

Tooth #18:
Deep caries reaching the pulp with a deep crown fracture.
The root tips appear close to the maxillary sinus floor on the panoramic view.
Extraction was completed with normal saline irrigation, gauze placement, and full post-operative instructions.

Tooth #48:
Deep caries with crown breakdown, and the tooth was mesially tilted.
Extraction was completed with normal saline irrigation, gauze placement, and post-operative instructions.

βœ”οΈ Deep curettage of the infected socket** was done to remove granulation tissue and ensure proper debridement.

This case demonstrates the extraction of multiple maxillary teeth:
β€’ Tooth #16 (remaining roots): retained and located close to the floor of the maxillary sinus.
β€’ Tooth #18 (upper right third molar – wisdom tooth): presented with a fractured, carious crown and curved roots in close proximity to the maxillary sinus.
β€’ Tooth #28 (upper left third molar – wisdom tooth): fractured crown partially covered by a gingival flap, requiring a minor operculectomy before extraction; the roots were markedly curved and close to the maxillary sinus.

✨ Despite these anatomical challenges, all extractions were performed smoothly and atraumatically, without the need for surgical bone removal.
