One of the most challenging orthodontic cases due to a severe skeletal Class III pattern, characterized by a protruded lower jaw and a complete anterior and bilateral posterior crossbite.
In cases like this, orthognathic surgery is usually the ideal treatment. However, many patients decline surgery and instead seek a simpler and relatively less expensive alternative.
To make the case even more challenging, both lower first molars had large restorations, chronic infections, and poorly performed root canal treatments, leaving them with a hopeless prognosis. Therefore, extracting these molars was the best option instead of removing the lower first premolars—which would normally be the preferred and more straightforward extraction pattern if the molars had been healthy.
Treatment began with rapid maxillary expansion (Hyrax appliance) to correct the constricted maxilla and eliminate the posterior crossbite.
Next, the entire lower anterior segment, together with all the premolars, was retracted backward to correct the anterior crossbite and achieve a functional occlusion.
And here is the final result…
A result even more beautiful than Spain’s winning goal in the World Cup final—worthy of a trophy of its own! 😅😇
Alhamdulillah, all praise is due to Allah, from beginning to end.
