Why do we retreat a root canal that doesn’t hurt? 🤔

Localized apical periodontitis often stays "silent" until it’s too late.

For this patient, we chose a non-surgical retreatment to preserve the natural tooth.

Using Hyflex Remover and WaveOneGold Small, I successfully shaped the canals.
To give the tooth a long-term future, I reinforced it with a fiber post and prepared it for its final crown. 👑

Diagnosis
• Severe pulpitis caused by deep caries
• Apical tissues clinically within normal limits

Clinical Protocol
✔️ Rubber dam isolation
✔️ Complete caries removal & access cavity preparation
✔️ Distal pre-endodontic wall intentionally preserved to facilitate access
✔️ Liquid rubber dam applied to reinforce isolation and prevent coronal leakage
✔️ Coronal pre-flaring to improve canal entry
✔️ Initial scouting and negotiation with C-Pilot ISO 8 to working length (19 mm), followed by ISO 10 and 12
✔️ Rotary instrumentation using **TruNatomy**, adapted for curved anatomy
✔️ Continuous irrigation using TruNatomy irrigation needle
✔️ Identification of inter-canal communication (MB2–D–P)
✔️ Obturation performed with warm condensation technique
✔️ Direct coronal restoration placed

Management of a Mandibular Bifid Canine

The presence of bifid roots or two separate canals in mandibular canines is considered a rare anatomical variation, with a low prevalence in the general population. Such complexities can significantly challenge endodontic treatment, making magnification and proper instrumentation essential.

The patient presented as an emergency, with swelling and discomfort in the area of tooth 4.3, which had already been accessed for drainage.

Clinical & Radiographic Findings:
• Severe tenderness to percussion and palpation
• Localized swelling adjacent to tooth 4.3
• Pre-operative radiograph revealed:
– Bifurcation of the mandibular canine
– Two distinct canals
– Apical radiolucency

Diagnosis:
Pulp necrosis & localized symptomatic apical periodontitis

Treatment Plan:
Non-surgical endodontic treatment followed by direct restoration

Clinical Steps:
1. Access cavity refinement using Endo-Z bur and ultrasonic tips to eliminate coronal interferences
2. Buccal canal located first
3. Further refinement allowed straight-line access to the lingual canal, which required careful manual scouting with hand files due to limited accessibility
4. Buccal canal shaping with AF Rotary 20/.04
5. Lingual canal managed with a modified technique: the rotary file was initially introduced manually (without the endomotor) to negotiate curvature, then connected to endomotor to complete shaping safely
6. Visual gauging for proper instrumentation
7. Irrigation protocol: 5.25% NaOCl & 17% EDTA
8. Obturation using the warm condensation technique
9. A dual-cure composite was selected for the direct restoration due to cavity depth (a post-operative air bubble was detected and will be corrected)
10. Final radiograph confirmed adequate obturation of both canals within the bifid root system

📋 Diagnosis: Severe Pulpitis (positive cold testing)
👑 Crown: Zirconia (MFP crown previously replaced)
🔍 Procedure: Endodontic treatment performed through the existing zirconia crown (higher difficulty)

1️⃣ Pre-op X-ray (taken before crown replacement)
2️⃣ Rubber dam isolation
3️⃣ Access cavity through zirconia crown - Endo Z bur was very useful to refine the access cavity + visibility enhanced by Microscope
4️⃣ Canal shaping and cleaning (ProTaper Ultimate .romania )
5️⃣ Obturation (CWC)
6️⃣ Post-op X-ray

✨ Pain relief achieved — avoiding crown removal and maintaining aesthetics and function.

Pacientul s-a prezentat cu durere intensă la rece — un semn de pulpită severă. Coroana era din zirconiu, recent înlocuită.

Am realizat tratamentul endodontic prin coroana existentă, cavitatea de acces definitivatã cu ajutorul frezei Endo Z și al microscopului dentar — combinația care ne-a oferit precizie și vizibilitate.

📸 Etape:
1️⃣ Radiografie preoperatorie
2️⃣ Izolare cu digă
3️⃣ Acces prin coroana dentarã
4️⃣ Preparare ProTaper Ultimate şi irigare
5️⃣ Obturare (CWC)
6️⃣ Radiografie postoperatorie

✨ Rezultatul: durerea a dispărut complet, coroana a fost păstrată, iar estetica și funcția au rămas intacte.

🔎 Diagnosis: Severe Symptomatic Irreversible Pulpitis (AAE–ESE 2025)
Chief Complaint: The patient reported pain and prolonged sensitivity to cold.

🔬 Treatment Plan: Conservative endodontic treatment performed under magnification.

♟️ ACCESS CAVITY was extended toward the INCISAL EDGE to improve straight-line access and canal detection.
Two distinct canals were located — the incidence of two canals in lower incisors can reach up to 40%, although radiographically it may appear as a single canal.

Rotary instrumentation with TruNatomy Prime 26.04v, allowing optimal shaping while preserving dentin.

Irrigation: NaOCl and EDTA under agitation for complete debridement of the narrow, communicating canal system.

Obturation: Continuous Wave Compaction (CWC).

Motivul prezentării: Pacientul a acuzat sensibilitate prelungită la rece.

🔬 Plan de tratament:
Tratament endodontic conservator realizat sub magnificație.

♟️ Cavitatea de acces a fost extinsă spre marginea incizală pentru a facilita localizarea canalelor și accesul direct.
Au fost identificate două canale distincte — incidența a două canale în incisivii inferiori poate ajunge până la **40%**, chiar dacă radiografic apare un singur canal.

Instrumentare rotativă cu TruNatomy Prime 26.04v pentru o preparare optimă și conservarea maximă a dentinei.
Irigare: NaOCl & EDTA

Obturare: Condensare Verticală la Cald (CWC).

Treatment - Non-surgical Root Canal Treatment

A pulp stone was identified within the pulp chamber, initially compromising effective intrapulpal anesthesia.
After complete removal of the calcification, anesthesia was successfully achieved, allowing smooth continuation of the procedure.

Instrumentation:
Glide Path: MANI Glide Finders→ProTaper Ultimate Slider
Rotary Shaping: AF Rotary Files, taper .04 — focused on conservative, anatomical shaping.

* Sodium Hypochlorite (NaOCl)
* EDTA
* Activation with EndoActivator

Continuous Wave Condensation (CWC)

Fiber post placement

Tooth 4.7 will serve as an abutment in the future prosthetic rehabilitation.



Diagnostic: Pulpită ireversibilă

Tratament: Tratament endodontic

În camera pulpară a fost identificată calcificarea țesutului pulpar, care a îngreunat inițial obținerea anesteziei intrapulpare eficiente.
După îndepărtarea completă a calcificării, anestezia a fost stabilită cu succes, permițând continuarea tratamentului fără disconfort.

Instrumentare
• Glide Path — *MANI Glide Finders* → *ProTaper Ultimate Slider*
• Preparare rotativă — *AF Rotary Files*, conicitate .04
  → abordare conservativă, cu respectarea anatomiei sistemului endodontic

• Hipoclorit de sodiu (NaOCl)
• EDTA
• Activare cu EndoActivator

Obturare - Continuous Wave Condensation (CWC)

Restaurare coronară cu pivot de fibră de sticlă

Dintele 4.7 va servi drept stâlp protetic pentru viitoarea restaurare definitivă.
