Ablația coroanelor metalo-ceramice necesită atenție, mai ales când avem dinți cu structură restantă fragilă sau reconstituiri coronare extinse.

Full-mouth rehabilitation doesn’t start with the teeth. It starts with proper planning—specifically, the Esthetic Plane and the Occlusal Plane.

These two planes play a fundamental role in:

• Anterior and lateral guidance
• Distribution of occlusal forces
• Integration of restorations with facial esthetics
• Tooth–lip relationships
• Long-term stability of the restorations

When these planes are properly coordinated, we can achieve a more stable and predictable occlusal scheme.

When it comes to predictable indirect restorations, it’s often not the ceramic itself that makes the difference. It’s how we manage the soft tissues before the final impression, scanning, and cementation.

These clinical images illustrate challenges we frequently encounter:

✅ Compromised teeth, subgingival margins, and tissue deficiencies
✅ The need for gingival stability and proper papillary architecture
✅ Definitive restorations that must integrate naturally with the surrounding tissues

What does soft-tissue management involve in prosthodontics?

1. Preparation margin placement
Margins should be accessible, maintainable, and biologically stable—not simply hidden beneath the gingiva.

2. Provisional restorations
Provisionals are more than temporary restorations. They help shape the gingival margins, support the papillae, and establish the emergence profile.

3. Emergence profile
Overcontouring can contribute to inflammation and recession, while inadequate contours may compromise tissue support and contribute to open gingival embrasures.

4. Final impression or scan
Timing matters. We need healthy, stable tissues, a clearly defined sulcus, and effective hemostasis before capturing the final impression.

5. Marginal adaptation and cervical contours
A restoration may fit perfectly on the model but still fail clinically if its margins and contours do not respect the surrounding periodontal tissues.

Predictable indirect restorations start with healthy, well-managed soft tissues—not just beautiful ceramics.

In indirect restorative dentistry, impression-taking is more than a technical step. It is fundamentally a matter of tissue management.

This case involves multiple anterior preparations, where proper soft-tissue management is essential to achieve:

• clear visualization of the preparation margins
• accurate reproduction of the emergence profile
• predictable marginal adaptation of the definitive restorations

🔹 Double-cord retraction

The first cord is placed deeper in the sulcus to assist with fluid control and protect the epithelial attachment.
The second cord provides lateral tissue displacement and adequate exposure of the preparation margin.

If the entire margin is not clearly exposed, the accuracy of the final impression may be compromised—regardless of whether we use a conventional impression material or an intraoral scanner.

🔹 Hemostasis and crevicular fluid control

A clean, dry field is essential. Blood or crevicular fluid around the preparation margin can interfere with accurate margin capture and ultimately compromise the fit of the definitive restoration.

🔹 Final impression / intraoral scan

The quality of the marginal detail in the final digital model does not come from the scanner alone. It begins with proper tissue management before scanning.

This is one of the principles behind the INDIRECT concept:
A restoration can only be as precise as the information we provide to the laboratory.

And that information depends, first and foremost, on proper biological and soft-tissue control.

Do you use the same retraction protocol for every case, or do you modify your approach based on the gingival phenotype and the position of the preparation margin?

In many cases, when working with indirect restorations, our attention tends to focus primarily on the restorative material. In reality, however, the success of a restoration always begins with the clinical protocol.

Controlling tooth preparation based on the available restorative space, proper soft-tissue retraction to clearly expose and accurately finish the preparation margins, precise adaptation of the provisional restorations, and, last but not least, meticulous isolation for adhesive cementation—these are the steps that ultimately lead to a predictable, functional, and esthetic outcome.

– evaluare estetică a suportului labial
– testare prin mock-up provizoriu
