We all see nervous patients. Remember the amygdala in the brain of our patients stores images which evoke positive and negative feelings. For many patients the sight of the chair or the smell of the surgery will evoke fear. Often consultation in another room which isn’t the surgery can help for nervous patients on the initial visit.

Carotid Sinus which is present near the bifurcation of the common carotid arteries have stretch receptors (baroreceptors ) in their walls.
A decrease in arterial pressure decreases stretch on the walls of the carotid sinus. Decreased stretch decrease the firing rate of Hering's nerve (carotid sinus nerve) (CN 1X), which carries information to the vasomotor centre of the brain.
The set point for mean arterial pressure in the vasomotor centre is about 100 mm Hg. A decrease will result in activation of autonomic responses and supression of parasympathetic outflow, that will try to raise blood pressure towards normal.

There is nothing magical about Calcium Hydroxide when used as a pulp capping material. It simply works by dissociating into Calcium (Ca+2) and Hydroxyl (OH-) ions. The Hydroxyl ion is bacteriocidal as it binds to the lipid transport protein on the cell membrane and creates a lipid free radical which is unstable causing the bacterial cell to fall apart (lyse). So when you use it as a pulp cap it prevents bacteria entering the pulp. The hope is that by the time the Hydroxyl ions reserve has been used there has been a dentine bridge formed over the pulp. The formation of a dentine bridge is the NORMAL response to the pulp being exposed. In the presence of bacteria the dentine bridge often can’t form, which is why CaOH is so useful.

But newer materials such as MTA and Bioceramics release Calcium Hydroxide whilst setting and then once set form a cement barrier to prevent bacterial ingress. This is why these materials are more reliable. They use both the antibacterial action of calcium hydroxide and the physical barrier - this is the main problem with setting calcium hydroxide which breaks down and therefore lacks the physical barrier required for reliability as a pulp capping material.

Parotid Gland is the largest salivary gland and is composed mostly of serous acini. The facial nerve divides the gland into the superficial and deep lobes. The parotid duct (Stenson's duct) emerges from the anterior border of the gland and passes forward over the lateral surface of the Masseter. It enters the vestibule of the mouth upon a small papilla opposite the upper 2nd Molar tooth.
Parasympathetic secretomotor supply arises from the glossopharyngeal nerve. The nerve reaches the gland via the tympanic branch, the lesser petrosal nerve, the otic ganglion and the auriculotemporal nerve.
