Case Description:

This patient presented with a chief complaint of persistent pain in the tooth for the past 7 days. Clinical examination revealed a grossly carious tooth 16 that was tender to percussion. Pulp vitality testing confirmed a non-vital pulp.

Based on clinical and diagnostic findings, a diagnosis of periapical periodontitis  secondary to pulp necrosis was established.

Treatment Procedure:

The palatal wall of the tooth was missing; therefore, it was rebuilt using composite resin to aid in proper isolation and rubber dam placement. Access cavity preparation was performed, and canal orifices were identified.

Working length was determined using an apex locator . Biomechanical preparation was carried out using NiTi rotary heat treated files. During canal preparation, while shaping the MB2 canal, a #20 NiTi file separated within the canal. The separated instrument was successfully retrieved using H-files.

Copious irrigation was performed throughout the procedure using sodium hypochlorite solution 6%

After completion of cleaning and shaping, the canals were dried with sterile paper points. Obturation was carried out using gutta-percha cones.
The access cavity was sealed with a permanent restoration.

Pain Free Root Canal treatment of tooth 36 at Dr Shahbaz Dental & Implant Centre Lahore.

This patient was referred to me by a general dentist to see if the tooth could be saved
Pt presented with complaint of severe pain in her tooth for the past 1 month. On examination the tooth was carious and tender to percussion .Pt was diabetic and tooth was grade 1 mobile Diagnosis of Apical Periodontitis secondary to irreversible pulpitis was made and root canal treatment was performed.
Distal wall was carious so composite wall was built for isolation purposes Rct was performed &
Cavit was placed and pt was sent back to the dentist for permanent restoration .
